Description

Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation VA-250-11-RQ-0435 is being issued as a Request for Quotations (RFQ) and the intent is to award a firm fixed price contract for supplies. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-18. This requirement full and open competition after exclusion of sources, small business set aside, for commercial items under the North Atlantic Industrial Classification System (NAICS) 334516 Analytical Instrument Manufacturing, size standard in number of employees: 500. Requirements: Network Contracting Activity 10 for The Louis Stokes Cleveland VA Medical Center (VAMC), 10000 Brecksville Road, Brecksville, Ohio 44141 is seeking quotes for Physiologic Instruments, or brand name or equivalent. The use of brand name or equivalent is not intended to be restrictive but descriptive. Contractors should include in their quote product information demonstrating salient features of the equipment. Line items with specifications are as follows: Line item 1: VCC MC8 Multichannel Voltage-Current Clamp w/ 8 clamp channels, Includes 8 DM_MC6 input modules with integral dummy membrane Line item 2: Easymount System for Cell Culture- Snapwell Inserts, Line item 3: P2308, 1.0mm diameter aperature Line item 4: P2300-CAP, Cap with sampling hole for P2250, P2300 and P2400 Chambers, w, left and right caps Line item 5: P2020-S, AG/AGCL electrode set for Easymount Chamber system Line item 6: P2023-100, Spare electrode tips for P2020 Line item 7: FN15, 15g, 3" soft tipped needle Line item 8: A&A II, Acquire and Analyze revision 2.0 Data Acquisition Software and hardware system Line item 9: TB-115, ThermoFisher AC150 S7 heating bath Line item 10: TK-7, Connection and Installation Kit for Ussing Chamber system Quotes with additional features will be considered, provided the minimum requirements of the Government have been met. Each line item must have a description and pricing information. Delivery: FOB destination is preferred. The government will arrange for shipping if the estimated cost for handling is over $250. Products shall be delivered 30 ARO, to 10000 Brecksville Road, Brecksville, Ohio 44141. NOTE: The Government reserves the right to award without discussions, vendors shall submit the most favorable terms. Invoices shall be submitted in accordance with Federal Regulations 52.212-4(g) and shall be submitted to Financial Management Systems, PO Box 149971, Austin, Texas 78714. Remittance address: Payments under this contract shall be made to the address indicated in your quote. Provisions and Clauses: The clause at 52.219-5 Notice of Total Small Business Set Aside is applicable to this procurement.The provision at 52.212-1, Instructions to Offerors- Commercial, applies to this acquisition. Award will be made on the basis of Lowest Price Technically Acceptable. Offerors should include a completed copy of the provision at 52.213-3, Offeror Representations and Certifications- Commercial items, with offer. The clause at 52.212-4 Contract Terms and Conditions-Commercial Items, applies to this acquisition, to include the following addendum to the clause: None. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items, applies to this acquisition which shall include the following clauses: 52.222-21, 52.222-26, 52.222-35, 52.22236,52.222-37,52.225-3. The preceding Federal Acquisition Regulations (FAR) clauses and provisions may be accessed on the Internet at http://www.arnet.gov/far. The preceding Veterans Administration (VAAR) clauses and provisions may be accessed on the Internet at http://www.va.gov/oa&mmlvarr/.
