- Washington University The Centers for Disease Control and Prevention intends to award on a single source basis a purchase order to Washington University, Center for Tobacco Policy Research(CTPR), St. Louis MO 63112-1408. The purchase order is to provide the following three objectives: • Conduct an analysis of chronic disease program data that is currently available to test the usability of the tool. • Develop and implement a training and technical assistance plan for programs across chronic disease areas. • Plan and implement dissemination activities, including developing a web-based sustainability assessment tool and presenting the tool at relevant meetings and conferences such as the state program managers meetings and national conferences. The Center for Tobacco Policy Research performed the initial ground breaking work on sustainability efforts for state and local goverments. State and local governments have unique characteristics/factors and the Center for Tobacco Policy Research has studied and evaluated these factors closely. Funding another entity would duplicate time and effort and therefore is not within the government's best interest.. The following specific project objectives are to be addressed by the contractor: The Center for Tobacco Policy Research will monitor and manage the creation of the sustainability tool and its dissemination and ensure project deliverables are completed on time: 1. CTPR will track the progress of deliverables throughout the performance period 2. CTPR will provide feedback on sustainability tools, reports, and plans as requested 3. CTPR will confirm the completion of deliverables CTPR will: 4. Obtain regular feedback from advisory committee, comprised of experts from academia, funding agencies, and federal, state, and community chronic disease programs 5. Analyze the psychometric properties of sustainability data that is currently available (e.g., secondary data sources) using Structural Equation modeling (SEM). Specifically, Confirmatory Factor Analysis, along with Cronbach's α will be used to determine the reliability. 6. Refine instrument based on analysis of secondary data from many chronic disease areas. 7. Develop and implement a training and technical assistance plan for utilization of the tool, including user kits and training DVDs 8. Develop a web-based assessment tool 9. Disseminate the tool through national conferences and other national meetings 10. Develop a final report 11. Develop a report or paper on community-level programs and state-level programs This order is a firm fixed price requirement. The contractor shall produce acceptable deliverables required under this order. Item Description Quantity Delivery Date 1. This requirement is not set aside for small businesses. It is being procured under the simplified acquisition procedures, found in the Federal Acquisition Regulations (FAR) Part 13. The authority for this sole source is FAR 13.106-1(b)- Solicitation Competition-single source. The period of performance shall continue for 12 months after award date. This notice is not a request for competitive proposals. However, firms that believe they can provide all items listed for procurement need to submit a written notification of interest and capability to the Contracting Specialist within 15 days from the publication date of this notice. A determination by the Government not to compete this proposed action based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether or not to conduct a future competitive procurement.
