- Performance Work Statement in regards to Fitness Center Equipment Maintenance This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation FA4877-12-Q-0070 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53, Defense Circular Number 20110819, Air Force Acquisition Circular 2011-0714. This procurement is being solicited as a 100% small business set-aside under NAICS code 811310 with a size standard of $7.0M. Quotes shall contain a price breakdown of the base year and four (4) option periods. DESCRIPTION: 0001 - 12 MO: Equipment Maintenance and Repairs 0002 - 1 LO: Parts as needed, reimbursable to the contractor *Contractors should not quote on CLIN 0002, as this is a parts reimbursable CLIN. - Base Year Period of Performance: 1 Oct 2011 - 30 Sep 2012 - Option Year One: 1 Oct 2012 - 30 Sep 2013 - Option Year Two: 1 Oct 2013 - 30 Sep 2014 - Option Year Three: 1 Oct 2014 - 30 Sep 2015 - Option Year Four: 1 Oct 2015 - 30 Sep 2016 Offeror must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, with its offer.
