SUBMIT QUOTES BY SEPT. 16, 2011 @ 4:30PM MST TO: CIBOLA NATIONAL FOREST ATTN: BRIAN COOPER 2113 OSUNA ROAD ALBUQUERQUE, NM 87113 ***FAXED QUOTES ARE ACCEPTED FOLLOWED BY A MAILED ORIGINAL SIGNATURE COPY. *** (505-346-3909 FAX#) AGGREGATE PLACEMENT LOCATION Placement location is of the estimated 9,602 CY of aggregate is approximately 46 miles north of Bernalillo, NM. Beginning at I-25/Exit 242, go approximately 24 miles on US 550 northwest of Bernalillo to NM4 at San Ysidro, 9.5 miles on NM 4 to NM 485, approximately 4 miles to Gilman, 2 miles on NFSR 376 to the end of the asphalt surfacing. Continue on aggregate portion for approximately 8 mles to Porter Landing Bridge. Beginning of project and spot resurfacing, on NFSR 376 for approximately 3.6 miles to the second Cebolla Creek X-ing. Placement of 6 inches loose material. Or as staked on the ground. National Forest System Road (NFSR) 376 Item 1 Description- This work consists of furnishing, hauling, and windrowing aggregate base, Grading 1-inch, to NFSR 376, also known as Lake Fork Canyon Road. Material- Furnish hard, durable particles or fragments of crushed stone or gravel conforming to the size and quality requirements for crushed aggregate, FP-03 Table 703-2 Column D (Base) Specifications, or material normally used locally in the construction and maintenance of highways by Federal or State agencies. Furnished crushed aggregate with a maximum size of 1 inch as determined by AASHTO T 27 and T 11. Furnish crushed aggregate uniformly graded from coarse to fine and free of organic matter, lumps or balls of clay, and other deleterious matter. Construction Requirements- Furnish aggregate and load aggregate into suitable haul vehicles. Haul the aggregate to the project site. Place the aggregate in a uniform windrow along the centerline of the road as directed. Keep all equipment within the existing roadway. Turn vehicles only at designated locations. Cultural Resource Protection- Do not excavate, remove, damage, alter, or deface any archeological or paleontological remains or specimens. Control the actions of employees and subcontractors on the project to ensure that protected sites are not disturbed or damaged. Should any of these items be encountered, suspend operations at the discovery site, and notify the Contracting Officer, District Ranger, Contracting Officer Representative, Archaeologist, Supervisor etc. and continue operations in the other area. The Contracting Officer will inform the Contractor when operations may resume at the discovery site. Follow all restrictions regarding construction time and off-road vehicle use. Acceptance- Funrish a production certification including gradation and quality properties for each source of aggregate, including weight per volume of aggregate test. Measurement- Measure crushed aggregate by the cubic yard in the hauling vehicle and provides weight ticket. Payments- The accepted quantities will be paid at the contract price per unit of measurement for crushed aggregate in the bid schedule. Payment will be full compensation for the work prescribed above. Evaluation Criteria- The award of this contract is subject to approval of the number of suitable trucks to complete haul in a timely manner. Contractor shall provide a minimum of ten belly dump trucks to haul aggregate Daily (or) minimum of 540 loose cubic yards hauled per day. Award of contract also subject to approval of material by visual inspection and approval of material testing reports. AGAR 452.211-72 Statement of Work/Specifications (FEB 1988) The Contractor shall furnish the necessary personnel, material, equipment, services and facilities (except as otherwise specified), to perform the Statement of Work/Specifications referenced in Section C.
