Description

Rural Health Outreach Clinic in Williamson, West Virginia and Logan, West Virginia This is a sources sought notice only; it is not a request for quotations or offers. The Huntington VA Medical Center is conducting market research to identify potential candidates with interest in providing the delivery and management of primary and preventative medical care for Veterans enrolled within the geographic area of Mingo County, West Virginia. A second geographic area to be served is Logan County, West Virginia. The parent facility for these Rural Health Outreach Clinics is the VA Medical Center in Huntington, West Virginia. The Mingo County, West Virginia clinic must be located within the city limits of Williamson, West Virginia. The second clinic should be located within the city limits of Logan, West Virginia. Both clinics should operate at least two days per week. The contract may include one or both locations. The contract period will be for one (1) year from the date of contract award, with four (4) one-year renewal options to be exercised at the discretion of the government. NAICS Code 621498 applies with a size standard of $10.0 M. The minimum essential services to be provided include: "Primary Care Services: Primary care services are to include, but are not limited to, the full continuum of care from prevention to diagnosis, treatment and management of medical diagnosis, health promotion (screening and counseling), management of acute and chronic medical illnesses, physical examinations, injections and immunizations and overall patient care management. Specialty care not available and all other non-emergency referrals should be directed to the Huntington VA Medical Center (parent facility). "Mental Health Services: Rural Health Outreach Clinic is to be able to provide an appropriate venue for patients to utilize Telehealth Mental Health services provided by the Huntington VAMC. Services are to include proper patient intake, provision of secure and confidential space for the Telehealth appointment, and a facility with capabilities to provide electronic Telehealth services. Minimum staffing requirements for one or both locations include: "Primary Care Provider (MD) "RN Care Manager "Clinical Associate (LPN) "Clerical Associate Clinical staff must meet appropriate credentialing, privileging, and licensing requirements as required by the state of West Virginia. Physician assistants and nurse practitioners may provide care under the strict supervision of the licensed independent physician. Facility must be a permanently located structure (non-mobile) and meet all ADA compliance standards. Past performance data for similar size facilities, multiple locations which demonstrate your capability to satisfactorily execute a contract to provide primary medical care services as described in this notice. Data shall include customer name, address, contract number, and point of contact information (name, phone number, and e-mail address.) 4. The business size and business socioeconomic category; such as Service-Disabled Veteran Owned Small Business, Veteran-Owned Small Business, Hub Zone Small Business, etc. Please note, all SDVOSB's and VOSB's must be verified by the office of Small Disadvantaged Business Utilization's Center for Veterans Enterprise (CVE) prior to receiving any award. Interested parties should respond not later than October 4, 2011 at 2:00 P.M. Central Time. This sources sought notice is to assist the Huntington VA Medical Center in identifying an appropriate acquisition strategy only. Written communication received will be considered solely for the purpose of determining current market capabilities for the services identified within this notice. A solicitation is not currently available. If a solicitation is issued, it will be announced and posted on this website at a later date, and interested parties must respond to the solicitation to be considered for award. This notice does not commit the government to contract for any supply or service. The government will not pay for any information or administrative cost incurred in response to this source sought notice. Do not contact medical center staff in regards to this requirement, as they are not authorized to discuss matters related to this procurement action. All questions should be submitted in writing for this procurement to the Contract Specialist.
