Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation quotes are being requested and a written solicitation will not be issued. The solicitation number, VA-262-11-RQ-1118, is issued as a request for quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53 effective August 4, 2011 and Veterans Affairs Acquisition Regulation Supplement (VAAR) current to January 2010. The North American Industry Classification System (NAICS) Code for this acquisition is 423450, size standard: 100 employees. This procurement is unrestricted. Offeror's quotes are hereby being requested and shall be submitted on offeror's letterhead or bid form and shall contain the name, title, and signature of person authorized to submit quotes on behalf of the contractor. The following items are manufactured by The Medlox Group of Essex Industries, Inc. and are known to meet the VA's needs. Offerors shall provide a quote for the following brand name or equal items: ITEM #1: Mass Oxygen Distribution System (MODS) 75 unit with fill harness assembly, Stock Number: 10C-00613, Quantity: 2 each, ITEM #2: Patient Distribution Kit (minimum 3 required), Stock Number: 50C-00921, Quantity: 10 each, ITEM #3: 100 Foot Oxygen Hose Assembly, Stock Number: 50C-0092003, Quantity: 8 each, ITEM #4: VGL-MODS Liquid Oxygen (LOX) Transfer Harness Kit (2 required), Stock Number: 50C-010115, Quantity: 4 each. Offeror shall quote the unit price and the extended price for each line item. SALIENT CHARACTERISTICS: MODS can store over 64,000 gaseous liters in liquid form. LOX then safely expands exponentially as it is warmed to a gaseous state. This allows the MODS to carry more than three-and-a-half times it's volume in the same size container as a high-pressure oxygen tank. In addition, the MODS trailer is designed and outfitted with storage areas for additional EMS equipment. MODS can supply oxygen to multiple patients at once. With the portable patient distribution kits (PDK's) (which are small and lightweight oxygen supply control centers) and the ample and lengthy oxygen supply hoses, the MODS can reach isolated and tight surroundings patient locations. Delivery time, which shall be proposed on the offeror's quote, shall be FOB DESTINATION to the VA Long Beach Healthcare System, Warehouse - Bldg. 149, 5901 East 7th Street, Long Beach, CA 90822. The Offeror is responsible for all freight charges. All items offered must comply with FAR 52.211-6 Brand Name or Equal and must have the like salient characteristics of the items stated above. The Offeror shall include technical documentation to support products being offered as equal items. If offering an equal product, Offeror must complete the following: Brand Name, and Make or Model Number. If offering an equal product, offer must include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and clearly describe any modifications the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modifications. The offeror shall also provide warranty provisions, and any training and or installation that will be required. Award will be made to the offeror representing the best value to the Government. The provision at 52.212-3, Offeror Representations and Certification--Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its quotation. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items. The Offeror shall also provide its DUNS number and Tax Identification number, address and phone number. As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award.
