Attachment J Attachment C Table 2 Attachment D Attachment K Attachment M Intention to Propose Request for Proposal Instructions Attachment C Table 1A Attachment I Evaluation Criteria Attachment C General Provisions LHT&M Sample Contract General Provisions Wage Determination Statement of Work Reps & Certs The services contemplated under this RFP are generally described as Occupational Health & Medical Services (Base Contract) with Options for a Staff Clinic (Option 3) and Limited Prescription Drug Services (Option 4). The Contractor shall furnish all personnel, equipment, material, supplies, Lower-tier Subcontracts, and services. The Contractor shall otherwise do all things necessary for, or incidental to, performing in an efficient and effective manner all work (hereinafter referred to as 'Work') set forth in the Statement of Work and Technical Requirements - Occupational Health and Medical Services with Options for a Staff Clinic and Prescription Drug Program (Attachment A). The Work under this Contract is comprehensive in that the Contractor is expected to perform all technical, operational, and managerial functions necessary to provide the Work.
