Description
This is a combined synopsis / solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION DOCUMENT OFFERED. QUOTATIONS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. This solicitation VA-246-11-RQ-0552 is issued as a request for quote (RFQ). Award will be made using simplified acquisition procedures in accordance with FAR part 13.5. No telephone requests will be accepted. Only written requests received directly from the requester are acceptable. It is the Offeror's responsibility to ensure that the quote is received. The associated North American Industry Classification System (NAICS) code for this procurement is 811219. REQUESTERS MUST STATE THEIR BUSINESS SIZE ON EACH REQUEST. This action is unrestricted. If offeror has a FSS/GSA contract, please include it on your offer. See below for a list of items required. The Department of Veterans Affairs, Salisbury, NC has a need for a Maintenance Service agreement on the following equipment: Spectris Solaris/EP DCB-SMRS MRI DCB-SMRS MRI Stellant D DCB-SCTD DCB-SCTD CT DCB-SCTD CT The annual Maintenance Service agreement needs to include the following: Predictive Maintenance (PM) Coverage: Calibration Certification Travel, Labor, and Parts Medrad Product Hardware and Software Updates PM Hours of 8am - 5pm (Monday - Friday) 90 Day Warranty on Work Performed Corrective Maintenance Coverage: 24x7 Telephone Support Full Service Warranty for Term of Agreement On-site Emergency Service Hours of 8am - 5pm (Monday - Friday) Full Parts Coverage for Term of Agreement The service period required is 10/1/2011 through 9/30/2012. We will also accept quotes for option year 1 (10/1/2012 through 9/30/2013) and option year 2 (10/1/2013 through 9/30/2014). Submitted quotations shall include the RFQ number, the proposed cost for each piece of equipment, for years proposed, company name, and point of contact, address, phone number, fax number, tax identification number, DUN number, and business size. Service address is: Salisbury VA Medical Center, 1601 Brenner Avenue, Salisbury, North Carolina. 28144. Attention: You MUST be Medrad trained and an authorized service provider of Medrad equipment. You must also be registered in CCR to be considered for award per FAR 52.204-7, Central Contractor Registration (April 2008) (b) (2) the offeror shall provide its DUNS, or if applicable, it's DUNS+4 number with its offer, which will be used by the contracting officer to verify that the offeror is registered in the CCR database. Offers will be accepted if received no later than 10:00 am (EST) on September 16, 2011. Quotations will be accepted by e-mail only. Send quotes to dori.dabbs2@va.gov. The following clauses and provision apply to this procurement: 52.212-4, Contract Terms and Conditions-Commercial, Commercial Advertising, 852.271-70, 52.212-5, Contract Terms and Conditions required to Implement Statutes or Executive Orders-Commercial, including clauses incorporated by reference under Section b (1)(5)(11-16)(23-24); Offerors are to submit pricing. Process of determining contract award: award will be to the lowest-priced responsive, responsible bidder who can provide service listed above. This information should be specific, concise, and complete and provide sufficient information to demonstrate the offeror's capacity to meet or exceed the requirements of the contract. The information required in provisions 52.212-1, 52.212-2, a completed copy of provisions 52.212-3, Offeror Representations and Certifications, Commercial Items, 852.219-70 apply.
