Description
RECOVERY - PER FAR 5.7 THIS NOTICE IS PROVIDED FOR INFORMATION PURPOSES ONLY; THEREFORE FAR 5.203 DOES NOT APPLY. THIS OPPORTUNITY WAS AVAILABLE ONLY TO CONTRACTOR(S) UNDER THE CURRENT CONTRACT NUMBER(S): DeMaria Building Company,3031 W. Grand Blvd Ste 624, Detroit, MI 48202-3008, W911XK-10-C-0003, Saginaw HVAC Contract Award Amount equals $386,894.91 Recovery Funds, and $110,246.61 Non-Recovery Funds. A. SCOPE OF WORK DT004 Auditorium South Wing Revisions 1. Existing Conditions/Demolition 1.1. The contractor is to demolish the existing, suspended, hard plaster ceilings in rooms 1-1376 (Storage), 1-1374 (TV Room), 281 (Storage), 282 (Police Services Training) and the Auditorium second floor hallway/stairwell ceilings. 1.2. The contractor is demolish approximately twelve feet, six inches (12'-6") of the existing wall between rooms 281 (Storage), 282 (Police Services Training), as shown on sketch DT04 SK-01. 2. Architectural 2.1. The contractor will also install a new acoustical ceiling grid system with new lights in rooms 1-1376 (Storage), 1-1374 (TV Room), 281 (Storage), 282 (Police Services Training) and the Auditorium second floor hallway/stairwell ceilings. The new ceiling will be white tile with white grid. 2.2. The contractor will install a new wall between the expanded Police Services Training Room and the new Storage Room, as shown on sketch DT04 SK-01. 2.3. The contractor is to modify the soffit/bulkhead on the 1st floor to accommodate the new supply and return duct. 2.4. The contractor is to expand the bulkhead as shown on sketch DT04 SK-01. 2.5. The contractor will repair the floor were the old wall was demolished and will install new 12"x12" CVT where the wall was removed. 2.6. The contractor will repair the floor where the new supply and return air ducts supply the second floor and install new 12"x12" CVT. 2.7. The contractor will install new 4" or 6" black base. 3. Mechanical 3.1. The contractor is relocate the new supply and return duct, as shown on sketch DT04 SK-01. 4. Plumbing 4.1. The contractor is to install a new wall hung lavatory, as shown sketch DT04 SK-01. 5. Communications 5.1. The contractor is to provide and install new conduit to the projector and speakers, as shown on sketch DT04 SK-01. Unless otherwise indicated, the conduit is to be a minimum of 3/4". 6. Electrical 6.1. The contractor is to install new lights, outlets, etc. as shown on sketch DT04 SK-01. 6.2. The contractor is to install new 2'x2' light fixtures in the grid in accordance with section 26 51 00 (INTERIOR LIGHTING). The lights shall be 2'x2' parabolic, 2 - U Lamps, Metalux by cooper lighting, model 2EP3GX-2U6T8S33I-UNV-EB81-U or approved equal. M1-374 (1-1374) (x4) M1-376 (1-1376) (x4) Auditorium second floor hallway (x1) 6.3. The contractor is to install nine (9) six inch (6") recessed can CREE CR6 LED in the modified Police Services Training Room. Contractor shall field verify work. DT024 Revised 4th Floor Construction Schedule (A) The Contractor is to revise the construction schedule to divide the 4th Floor of Building 1 into two separate phases. (i) The first phase will be Building 1, 4th Floor, Area 1 (Clinic and Purple Team). (ii) The second phase will be Building 1, 4th Floor, Area 2 (Gold Team, Office Areas and Audiology). (B) All other base contract requirements remain the same. DT031 4th Floor Window Pockets A. This is issued in response to RFI-0019 and addresses conflicts between proposed ceiling height and the top of the existing windows. B. The contractor shall provide and install window boxes surrounding the window openings on the 4th floors. The window opening shall surround the window opening 2" above the top of the window opening, 2 " on each side of the window opening, and extending 8" from the existing wall. C. The contractor shall use fine veneer plywood and paint to match the room color. D. Materials for the 4th floor window pockets were incorporated into change DT018-2, mod P00007. This change is for the labor to build and install the boxes. DT048 4th Floor Communications Hub Closet (A) Demolish all existing cable tray on 4th floor of Building 1 as shown on the attached drawing. (B) Install new zinc coated 4"x12" and 4"x6" basket type cable tray with center trapeze supports, rollers at 90 degree bends, grounding, wire guards, anchors and all other hardware to provide a complete installation. The new cable tray shall be located as identified on attached drawing. Field verify location of new cable tray with COR and VAMC. (C) Furnish and install new wiring and re- connect existing house phones located in 2nd floor corridor near rooms 1-4148 and 1-4230. (D) Furnish and install new wiring and house phones in Building 1, 4th floor corridor near rooms 1-4172, 1-4260, 1-4105 and 5th floor corridor near 1-5200. All locations are general and should be field verified. Contractor shall locate phones on stud walls to reduce channeling, if possible. Field verify location of new cable tray with COR and VAMC. (E) Furnish and install 1" general purpose inner duct in newly installed cable tray from hub room to end of corridor. There shall be one (1) inner duct per corridor or 4 total. All inner duct shall have pull string installed. DT050 Dietetics Storage Provide 3 8'x20' temporary, portable storage containers for 2nd floor Dietetics Dry Storage for a duration of 4 months. Verify placement location with the VAMC and COR. B. CHANGE IN CONTRACT DRAWINGS DT004 Auditorium South Wing Revisions Drawing DT04 SK-01 1. Building 1 Fourth Floor Area 2 (Gold Team, Office Areas and Audiology) will be constructed second while Area 1 is occupied.
