- FAR CLAUSES RFQ-12-007 SOCIAL WORKER, Notice to Quoters, SF-18 and Statement of Work CONTRACTOR REQUIRED FORMS TECHNICAL EVALUATION FACTORS This is a combines synopsis/solication for commercial items prepared in accordance with the format in the FAR Subpart12.6, as supplemented with additional information included in the notice. This procurement is being conducted under the Simplied Acquisition procedures pursuant to the Authority of FAR Subpart 13.5, Test Programs for Certain Commerical Items (10 U.S.C. 2304(g) * 2305 & 41 U.S.C. 253(g) and 253a 253b), FAR Part 12 Acquisition of Commerical Items (Title VIII of the Federal Acquisition Streamlining Acot of 1994 (Public Law 103-355)), and FAR f 37.4 Non-personal Healthcare Services (10 U.S.C. 2304 and 41 U.S.C. 253). This announcement constitutes the only solicitation; therefore, a written solicitation will not be issued. The Aberdeen Area Indian Health Service (IHS) intendeds to award a firm-fixed price requirement type contract in accordance with the Request for Quotes (RFQ) 12-007 for a Social Worker to perform services at the Spirit Lake Health Center in Ft. Totten, ND. The solicitation documents & incorporation provisions & clauses are those in effect through Federal Acquisition Circular 2005-44. The associated North American Industry Classification System code is 621330 and the small business size standard is $7 million. This is a small business set aside. In the event of conflict between any terms of the agreement and the Requirements Contract, applicable Federal laws and statues will prevail. The Requirements Contract does not obligate any Funds. Funds will be obligated as individual task orders are placed hereunder. Prior to the time that services are needed an individual task order will be be placed against this Requirements type contract and shall be submitted electronically, facsimile or hard copy.
