- Section K Reps. & Certs., Section L- Instructions., Section M-Evaluation Factors Section J, Applicable Documents, Wage Determination SOW, Pricing, Clauses Synopsis: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 11-247-SOL-00078 and issued as a request for proposals (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-38. This requirement is a 100% Small Business Set Aside with a North American Industry Classification (NAICS) code of 813920 with a size standard of $14M. The Department of Health and Human Services, Indian Health Service (IHS) has a requirement for an experienced and professional consultant to assist with implementation of the IHS Improving Patient Care (IPC) Model for Improvement at the Phoenix Area IHS Colorado River Service Unit, Parker (La Paz County), Arizona. The Government contemplates award of a firm fixed price contract resulting from this solicitation. Base + 1 option year. All offerors shall submit a completed version of 52.212-3 Offeror Representations and Certifications & 52.209-5 Certification Regarding Responsibility Matters (attached). Vendors must be approved and registered on the Central contractor Registration (www.ccr.gov) before award can be made.
