Description
Statement of Work - VOIP Study This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This acquisition will be conducted in accordance with FAR Subpart 13.5. This solicitation is a request for quotation (RFQ). The solicitation document incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-53 All offerors will be responsible for obtaining related amendments to this solicitation, if any, from www.fbo.gov. It is the responsibility of each potential offeror to monitor www.fbo.gov for any amendments or other information related to this solicitation. Any communications regarding this acquisition must be made in writing and forwarded via email to janine.kerns@nist.gov and shall identify the solicitation number, company name, company address, as well as point of contact email address and phone number. This solicitation is issued as a Request for Quotation (RFQ), to acquire the following: A Study and Analysis of the NIST Gaithersburg, MD and Boulder, CO campuses current telecommunications system and what NIST will require to upgrade its current telecommunications system to Voice Over IP (VOIP). The associated North American Industrial Classification System (NAICS) code for this procurement is 541690 with a size standard of $25.0 Million. This acquisition is being conducted as a total small business set-aside. Only offers from small businesses shall be considered. Contract Line Items: The contractor shall provide a firm-fixed price quotation for the following: CLIN 0001 NIST VOIP Study in Accordance with the Requirements of the attached Statement of Work. Quantity 1, Unit Each. APPLICABLE PROVISIONS AND CLAUSES FOR THIS SOLICITATION: The following provisions and clauses apply to this acquisition and are hereby incorporated by reference. If paragraph (j) of the provision is applicable, a written submission is required. ADDENDUM TO PROVISION 52.212-1 - QUOTATION SUBMISSION INSTRUCTIONS (b) Submission of quotations. Submit signed and dated quotations to the office specified in this solicitation at or before the exact time specified in this solicitation. Quotations may be submitted on the SF 1449, letterhead stationery, or as otherwise specified in the solicitation. As a minimum, quotations must include- (1) The solicitation number; (2) The name, address, and telephone number of the offeror; (3) Terms of any express warranty; (4) FOB Destination-based firm-fixed-price quote for CLIN 0001 (5) Technical Volume Response that contains the following: technical approach/work plan to fulfilling the requirements of this solicitation; a description of the offeror's previous experience in fulfilling the requirements of this solicitation to include agency name, description of work performed, date(s) of performance, and total dollar amount; and resume (s) of offeror personnel intended to work under the subject order; (6) If the quotation is not submitted on the SF 1449, include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. The Quotations must also acknowledge all Amendments. Quotations that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. (c)Period of acceptance of Offers. The offeror agrees to hold the price in its offer firm for 30 calendar days from the dates specified for receipt of offers. All quotes shall be received not later than12 :00 PM Eastern Time on September 19, 2011 at janine.kerns@nist.gov. Faxed quotes will not be accepted. EVALUATION FACTORS FOR AWARD: The Government intends to award a purchase order to the responsible Offeror whose technically acceptable quotation represents the lowest price to the Government. Only quotations that fully address and meet the technical requirements will be considered for award.
