Description
This is a combined synopsis / solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and supplemented with additional information included in this notice. The solicitation number is VA-244-11-RQ-0747 and it is issued as a Request for Quotation (RFQ).This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. The NAICS code is 561110 Successful offer will be the one that provides: (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Past performance Electronic Contract Management System Certificate Active NACI Background Technical and past performance, when combined, are Significantly more important than cost. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.. The VA Pittsburgh Healthcare System has a requirement for the commodities listed below. All work will be performed at our facility located at 1010 Delafield Road Pittsburgh PA. All communication regarding quotes, questions, etc. shall be submitted by email to Jesse C Rumbel (Jesse.Rumbel@VA.Gov). Phone calls will not be accepted. Quotes are due by 4 PM, Eastern Time, on 9/12/2011. Quotes shall be submitted by email. Please read the entire solicitation. Candidate will receive on the job training for each software program. 3.Perform general or routine duties, as requested oShall have basic computer skills and experience with Microsoft Office software applications including MS Word and MS Excel oShall be able to prepare correspondence and documents using the correct procedures; and knowledge of grammar, spelling, capitalization, punctuation and office terminology is required oShall be able to operate a computer, printer, scanner, fax machine and other office related equipment oShall be able to pass background screening/Special Agreement Check (SAC) performed by human resources at the VA Pittsburgh Healthcare System. oSelected candidate shall complete computer access and privacy training and security training oShall comply with HIPPA privacy requirements as well as VA computer security requirements oDo to the emergent need the contractor shall already have a NACI background investigation or NACI; NACLC; BI completed prior to solicitation closing o(a) The Government and the Contractor understand and agree that the services to be delivered under this contract by the Contractor to the Government are non-personal services and the parties recognize and agree that no employer-employee relationship exists or will exist under the contract between the Government and the Contractor's employees. It is therefore in the best interest of the Government to afford both parties a full and complete understanding of their respective obligations. (b) Contractor personnel under this contract shall not: (1) Be placed in a position where they are under the supervision, direction, or evaluation of a Government employee. (2) Be placed in a position of command, supervision, administration or control over Government personnel, or personnel of other contractors, or become a part of the Government organization. o (3) Be used in administration or supervision of Government procurement activities. Performance Period: The performance period will start at or around September 19th 2011, at 8 a.m. Eastern Standard Time and shall continue for a period of 144 hours; however, there is the option to extend based on the department needs and availability of funding on a 144 hour basis up to two more times. Operational Requirements: 1.Hours of Duty - approximately 24 hours per week, ___________ through _____________ 8:00 am to ___________ with flexibility depending on workload, but not to exceed 24 hours per week. 2.Transportation is the responsibility of the selected candidate(s). Parking is available at this location. 3.The contracted employee will not have any signature authority or the authority to obligate funds or approve leave, etc. 4.Overtime work may be worked by the contractor only to the extent it is specifically authorized in writing in advance by the contracting officer. No additional hours of overtime shall be worked without additional written authorization. 5.Contractor will be paid only for hours worked. In the event of unanticipated leave or business closures, such as: leave due to an at-work fire occurrence or an at-work allotment of administrative holiday leave, the VA will pay the contractor for the full day's work. However, in cases which necessitate leave due to occurrences such as inclement weather, or VA long-term office closure, the VA shall not provide payment to the contractor. General Information: 1.Shall maintain the highest standard of integrity and mutual respect during interactions within the department and other VA employees and customers. 2.Contractor's employee must be present at VA and must be actually performing the required services for the period specified in the contract or the contract cost will be decreased accordingly during each billing cycle. The adjustment will be equal to the contractor hourly rate times the hours the contractor's employee failed to provide the required services. 3.Contractor's employees must comply with all VA's policies and regulations, including Policy #HR-22C, Dress Code/Professional Appearance for Medical Center Employees 4.Holiday time is defined as any time worked on a legal Federal holiday. Legal Federal holidays for the purpose of this contract are listed as follows: New Year's Day, Martin Luther King's Birthday, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day If a holiday falls on Sunday, the following Monday will be observed as the National Holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a National Holiday by U.S. Government agencies. Also included would be any day specifically declared by the President of the United States of America as a National Holiday. 5.The contractor has certain rights and responsibilities. The contractor has the right to expect fair and considerate treatment, decent working conditions, and a sincere concern on the part of VA for the contractor as an individual. In return, VA expects that the contractor will serve diligently, loyally, and cooperatively. The contractor must avoid misconduct and other activities that conflict with the employment; exercise courtesy and dignity; and otherwise exhibit conduct, both on and off duty, in a manner that reflects positively upon the contractor and VA. 6.The contractor shall provide the appropriate employee to perform the prescribed functions. The basis of the tasks of this SOW require accumulated knowledge, therefore substitution of contract employees is greatly discouraged. No personnel substitution will be permitted unless such substitutions are necessitated by an individual's sudden illness, death or termination of employment, or conditions beyond the control of the contractor. In any of these events, the contractor shall propose substitute personnel who have qualifications that are equal to or exceed the qualifications required for performance as specified in this performance work statement and who are subject to approval by the departmental supervisor.
