Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, and in conjunction with FAR Subpart 13.5 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. 1. Solicitation number F3PT42121A002 is issued as a Request for Quotation (RFQ). 2. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition circular 2005-53 and DFARS Change Notice 20110819. 3. The associated NAICS code is 511120; size standard is 500 Employees. 4. See the attached Schedule of Supplies/Services (Reference Atch 1) for applicable contract line items (CLINS), quantities, and units of measures. 5. This requirement for Fujitsu Scanner Maintenance is in support of the Air Force Personnel Center (AFPC). 6. See the attached Schedule of Supplies/Services for dates and location of services. (Reference Atch 1 & 2). 7. The provision at 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. 8. The Government will award a firm fixed price contract resulting from this RFQ for Fujitsu Scanner Maintenance to the responsible offeror whose offer conforming to the solicitation provides the best value to the Government. Best value means the expected outcome of an acquisition that, in the Government's estimation, provides the greatest overall benefit in response to the requirement. In determining the best value the Government shall consider the following three factors: Conformance to Schedule of Supplies/Services, Price and Past Performance. The provision at 52.212-2, Evaluation - Commercial Items, applies to this acquisition. Therefore, submission of offers shall address all requirements contained therein with particular attention to paragraph (a) of the clause - contractors shall provide clear evidence of the understanding and ability to comply with the PWS and provide sufficient information demonstrating the company's technical capabilities and experience, including past performance references and points of contact for each project of similar scope. All information shall be submitted with an offeror's quotation. ADDENDUM TO FAR 52.212-2 Reference FAR 52.212-2, paragraph (a) is hereby tailored as follows: a. Conformance to the Schedule of Supplies/Services (Technical Capability). The technical response shall demonstrate the ability to provide the services in accordance with (IAW) the attached Performance Work Statement (PWS). Response shall demonstrate understanding of contract requirements. b. Price. Contractors are required to complete the attached Schedule of Supplies/Services (Reference Atch 1). Price will be evaluated on total aggregate. c. Past Performance. Past performance will demonstrate contractor/vendor's ability to provide the service IAW the requirements listed in the PWS. Provide at least one (1) and not more than two (2) references (either Federal or commercial). Furnish the following information for each reference listed. (i) Company/Division name (ii) Service (iii) Point of Contact name, email, phone number (iv) Contract number (v) Contract Dollar Value (vi) Period of Performance The following information shall be provided as part of the RFQ package. Past performance will be rated as acceptable/unacceptable based on the response from the references provided. The government reserves the right to utilize past performance information gained from other sources (i.e., past performance databases, other government sources, etc) to aid in evaluating past performance. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (c) through (o) of this provision. (a) Definitions. As used in this provision-- "Economically disadvantaged women-owned small business (EDWOSB) concern" means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States and who are economically disadvantaged in accordance with 13 CFR part 127. The term does not include glancing at or listening to a navigational device that is secured in a commercially designed holder affixed to the vehicle, provided that the destination and route are programmed into the device either before driving or while stopped in a location off the roadway where it is safe and legal to park. (b) This clause implements Executive Order 13513, Federal Leadership on Reducing Text Messaging While Driving, dated October 1, 2009. (c) The Contractor should- (1) Adopt and enforce policies that ban text messaging while driving- (i) Company-owned or -rented vehicles or Government-owned vehicles; or (ii) Privately-owned vehicles when on official Government business or when performing any work for or on behalf of the Government. Air Force Personnel Center (AFPC) require contractor support to provide maintenance for Fujitsu scanners. Automated Records Management System (ARMS) is the lifecycle record management system implemented to capture all records from inception to retirement of personnel. These scanners are an integral part of the management process.They are used to support the Automated Records Management System (ARMS). Maintenance services are required for all of the Fujitsu scanners listed on Attachment 2. 2.0 SCOPE/DELIVERABLES: 2.1. Contactor shall provide equipment maintenance and repair support for the Fujitsu scanners located at Randolph Air Force Base, TX. The Contractor shall provide 8x5xNBD. Items under this service level are covered during the PPM. The contractor shall respond the Next Business Day from notification from the Government that maintenance services are required. The contractor shall provide service during the Primary Period of Maintenance (PPM) as required by the Government. The PPM is defined as 0830 through 1630, Central Standard Time, Monday through Friday, excluding Federal holidays. At a minimum, the contractor shall provide maintenance support that shall include the following levels of support: • Unlimited telephone support from 5:00 a.m. - 5:00 p.m. PST • Guaranteed Next Business Day response time • Access to a help desk that tracks, monitors, and records all calls for quality assurance • A trouble-ticket management system that will track tickets from their creation until it is resolved • Parts not available from the technician while on-site during the maintenance call must be shipped the same day when service is performed. • A trained and certified technician will be dispatched to arrive on-site next business day • One free preventative maintenance and system review visit per year by a certified technician • Develop a comprehensive list of covered Fujitsu scanners and their service address with the assistance of the designated representative 2.2. The contractor shall develop and provide to the government documentation that clearly identifies what level of maintenance will be provided to AFPC for its maintenance visits. This documentation should also include a projected date for an annual no-cost maintenance and system review visit from the contractor. In addition, the contractor shall provide documentation that reflects projected costs for replacement parts of equipment items that will not be repaired at any cost due to the maintenance contract, if any. 2.3. The contractor shall provide AFPC a record documenting any repair or maintenance actions performed. The record should clearly identify the following items at a minimum: what item was repaired, the reason for the repair, the date, time and location of the repair and any associated cost. 2.4. The contractor's proposed personnel, at a minimum, shall have the following expertise or experience: 3.1.1. Personnel shall have expertise in Fujitsu scanner equipment repair and servicing. 3.1.2. Personnel shall adhere to DoD policy and procedures related to PWS requirements. 4. DELIVERABLES: Contractor shall provide: 4.1. A detailed outline of all items covered within the annual maintenance plan, to include an annual schedule of projected on-site preventative maintenance. Annual maintenance plan shall be provided along with the Contractor's response to the solicitation. 4.2. Direct line telephone support numbers, as well as other available online methods of submitting maintenance service calls, and contact information for a main Point Of Contact (POC) for maintenance inquiries and requests for AFPC are to be submitted along with the Contractor's response to the solicitation. 4.3. Hard copy receipts of any on-site maintenance support requests with details on work performed, no-charge parts used, and the cost if any of the consumables used to perform maintenance on the scanner during the course of the maintenance call. 5. LOCATION: The contractor shall be responsible for providing maintenance and repair support on equipment (identified in Attachment 2) located at the following locations: Air Force Personnel Center Automated Records Management Randolph AFB, TX 78150 6. TRAVEL: It is not anticipated that travel outside of the San Antonio based AFPC facilities will be required. 7. RESPONSIBILITIES OF THE GOVERNMENT 7.1. Government personnel shall not attempt repairs to scanners, unless agreed to by the Contractor. 7.2. Subject to security regulations, the Government shall permit access to the equipment which is to be repaired. 8. GUARANTEE/WARRANTY OF REPAIR SERVICE AND REPAIR PARTS All repair work shall be guaranteed/warranted for a period of ninety (90) calendar days. All parts, furnished as spare parts will be guaranteed/warranted, in accordance with the applicable manufacturer's warranty. 9. DISCONTINUANCE: 9.1. The Government reserves the right to discontinue maintenance and support services by giving the contractor thirty (30) days advance written notice or a shorter notice when agreed to in writing by the contractor. 9.2 The Government may extend or cancel the original discontinuance notice with a written notice to the contractor, provided such notice is furnished at least seven (7) days prior to the stipulated discontinuance date. 9.3. This discontinuance shall be accomplished at no cost to the Government. 10.0 SECURITY AND SAFETY: 10.1.1. Obtaining and Retrieving Identification Media. As prescribed by the AFFAR 5352.242-9000, Contractor access to Air Force installations, the contractor shall comply with the following requirements: a. No later than three working days prior to contract commencement, the contractor shall submit a written request on company letterhead to the program manager listing the following: contract number, location of work site, start and stop dates, and names of contractor employees needing access to the base. The authorized program manager will endorse the request and forward it to the issuing base pass and registration office or security forces for processing. Contractors will present government (state or federal) issued ID, and INS Form 9 (I9), before being issued a pass to enter the installation. At the time of issuance, a Wants and Warrants check is conducted for every contractor. Personnel employed by the contractor must get a pass for their privately owned vehicles at Bldg. #1032, Visitor Reception Center, with proof of following: (1) Liability Insurance (2) Current License Plates (3) Current State Inspection Sticker (If Required) (4) Valid State Driver License (5) A phone number for sponsor on base b. Vehicles owned by the contractor with the company names permanently printed on them are not required to obtain a pass as long as a current work order is presented at the time of entry. However, current liability insurance, state inspection sticker, and registration is required. The person driving the vehicle must have a valid operator license for the type of vehicle. c. The contractor is responsible for ensuring employees report to Bldg. # 1032, Visitor Reception Center, to present their Form I-9 (Employment Eligibility Verification). d. Upon completion or termination of the contract or expiration of the identification passes, contractors shall ensure that all base identification credentials issued to contractor employees are returned to the issuing office. If a contractor employee has been terminated, the credentials will need to be retrieved and returned to issuing activity so that employee does not have base access. If the credential is not retrieved then SF will need to be notified so base access is not allowed. e. Failure to comply with these requirements may result in withholding of final payment. 10.1.2. Personnel Security. Contractor personnel requiring unescorted entry to controlled or restricted areas designated by the installation commander shall comply with base access requirements. These requirements are contained in AFI 31-101, for installation access and AFI 31-501, for suitability determinations. 10.1.3. Listing of Employees. The contractor shall maintain a current listing of employees. The list shall include employee's name, social security number. The list shall be validated and signed by the company Facility Security Officer (FSO) and provided to the contracting officer and ISPM prior to the service start date. Updated listings shall be provided when an employee's status or information changes. 10.1.4. Freedom Of Information Act Program (FOIA). The contractor shall comply with DoD Regulation 5400.7-R/Air Force Supplement, DoD Freedom Of Information Act Program, requirements. The regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding For Official Use Only (FOUO) material. The contractor shall comply with AFI 33-332, Air Force Privacy Act Program, when collecting and maintaining information protected by the Privacy Act of 1974 authorized by Title 10, United States Code, Section 8013. The contractor shall remove or destroy official records only in accordance with AFMAN 33-363, Management of Records, or other directives authorized in AFI 33-322, Records Management Program. 10.1.5. Traffic Laws. The contractor employees shall comply with base traffic regulations.   Attachment 4 52.222-48 EXEMPTION FROM APPLICATION OF SERVICE CONTRACT ACT TO CONTRACTS FOR MAINTENANCE, CALIBRATION, OR REPAIR OF CERTAIN EQUIPMENT CERTIFICATION (FEB 2009) (a) The offeror shall check the following certification: CERTIFICATION The offeror ( ) does ( ) does not certify that-- (1) The items of equipment to be serviced under this contract are used regularly for other than Government purposes, and are sold or traded by the offeror (or subcontractor iin the case of an exempt subcontractor) in substantial quantities to the general public in the course of normal business operations; (2) The services will be furnished at prices which are, or are based on, established catalog or market prices for the maintenance, calibration, or repair of equipment. (i) An "established catalog price" is a price included in a catalog, price list, schedule, or other form that is regularly maintained by the manufacturer or the offeror, is either published or otherwise available for inspection by customers, and states prices at which sales currently, or were last, made to a significant number of buyers constituting the general public. (ii) An "established market price" is a current price, established in the usual course of trade between buyers and sellers free to bargain, which can be substantiated from sources independent of the manufacturer or offeror; and (3) The compensation (wage and fringe benefits) plan for all service employees performing work under the contract are the same as that used for these employees and equivalent employees servicing the same equipment of commercial customers. (b) Certification by the offeror as to its compliance with respect to the contract also constitutes its certification as to compliance by its subcontractor if it subcontracts out the exempt services. If the offeror certifies to the conditions in paragraph (a) of this provision, and the Contracting Officer determines in accordance with FAR 22.1003-4(c)(3) that the Service Contract Act-- (1) Will not apply to this offeror, then the Service Contract Act of 1965 clause in this solicitation will not be included in any resultant contract to this offeror; or (2) Will apply to this offeror, then the clause at 52.222-51, Exemption from Application of the Service Contract Act to Contracts for Maintenance, Calibration, or Repair of Certain Equipment--Requirements, in this solicitation will not be included in any resultant contract awarded to this offeror, and the offeror may be provided an opportunity to submit a new offer on that basis. (c) If the offeror does not certify to the conditions in paragraph (a) of this provision-- (1) The clause in this solicitation at 52.222-51, Exemption from Application of the Service Contract Act to Contracts for Maintenance, Calibration, or Repair of Certain Equipment--Requirements, will not be included in any resultant contract awarded to this offeror; and (2) The offeror shall notify the Contracting Officer as soon as possible, if the Contracting Officer did not attach a Service Contract Act wage determination to the solicitation. (d) The Contracting Officer may not make an award to the offeror, if the offeror fails to execute the certification in paragraph (a) of this provision or to contact the Contracting Officer as required in paragraph (c) of this provision.
