Description
MODIFICATION 003 BEGINS The purpose of this modification is to acknowledge Amendments 001 and 002 to this solicitation. Both amendments responded to questions which are presented herein as a consolidated list. Brand names, when shown, are included only to illustrate the desired item type. Proposals must identify the make and model number description of all proposed items whenever a brand name is shown. QUESTIONS Questions 1-9 are included in SF 30 dated 9/9/11 - Amendment 002 Questions 10-15 are included in SF 30 dated 9/8/11 - Amendment 001 1.Will the government accept a substitution of metal frame metal doors for the metal frame wood doors specified? Answer: No. Metal frame wood doors are required to match doors in the surrounding area. 2.(Section 08 11 13) Are the door frames welded frames or knock down frames? Answer: Knock down frames are not acceptable. (2.3, A., item 7.) 3.(Section 08 71 00) What Type of hardware do the doors get (pass thru, locking, push button, closers, kick plates, etc.)? Answer: Storeroom lock style, lever handle, locking door with LCN 1461 door closure or equivalent. No kick panels needed. 4. (Section 09 22 16) Are the walls sound insulated? No. 5.(Section 09 22 16) ½" or 5/8" type X gypsum board on walls? Answer: 5/8". 6.(Section 09 22 16) Are plumbing walls in the locker room framed with 3 5/8" or 6" metal studs? Answer: 3-5/8". 7. (Section 22 05 11) Locker room sinks size and type? Answer: White 18" x 20" wall hung lavatory sink. American Standard Lucerne or equivalent. 8.(Section 22 05 11) What type of faucets on sinks in locker rooms? Answer: Chicago Faucet 797-317XKCP, 4" on center, 4" wrist blade or equivalent. 9.(Section 22 05 11) What type of shower heads? Answer: American Standard 1660.731 or equivalent. 10.Where is the main sewer line to tie in the showers? Answer: There is a 4" sewer line in the area behind the water fountain. 11.Will the paint be one color for the entire job? Answer: Two colors in each room consisting of one solid color on three walls and a solid accent color on one long wall. 12.Is moving heat detectors required? Answer: No. 13.Does the new wall go up to the deck above for security or as a fire barrier? Answer: For security only. 14.Is the shower ceiling sheetrock or suspended ceiling? Answer: Sheetrock ceiling in the two shower/change areas. 15.Can the rubber flooring be installed over the existing 12" x 12" tiles? Answer: No, the flooring in exercise area includes removing the existing tiles and installing the rubber flooring per the manufacturer's recommendations. (See attached link). Proposals must be received not later than 3:00 pm Wednesday, September 14, 2011. Offerors must acknowledge receipt of all amendments in Block 19 of the SF 1442 submitted with their proposal. MODIFICATION 003 ENDS MODIFICATION 002 BEGINS The purpose of this modification is to issue the solicitation notice and change the time of the site visit. The site visit is scheduled for Friday, September 2 at 1:00 pm. Please note the time is changed from the previous notice. Participants meet at the second floor conference room in Building 17 at the intersection of 22nd Street and Garfield, Sioux Falls, SD. Contact Jeff DeYong at tel 605-226-3230 to confirm the meeting date, time and location. All questions must be submitted in writing to the issuing office via email to Kathryn.Crowthers@va.gov and copied to Michael.Reed1@va.gov. The last day to submit questions pertaining to this solicitation is close of business (4:00 pm) Wednesday, September 7, 2011. Proposals are due not later than 3:00 pm CDT Monday, September 12. Anticipated award date is September 24, 2011. MODIFICATION 002 ENDS MODIFICATION 001 BEGINS The purpose of this modification is to clarify the presolicitation notice posted August 15, 2011. Drawings and detailed requirements will be posted with the solicitation. No specifications or drawings are available until the solicitation issued. The solicitation notice is scheduled to be issued August 30, 2011 and proposals will be due September 12, 2011. Z - MAINTENANCE, REPAIR OR ALTERATION OF REAL PROPERTY Z - CONSTRUCTION - Construct Fitness Center Proj #438-11-119 THIS IS A TOTAL 100% SERVICE DISABLED VETERAN OWNED SMALL BUSINESS (SDVOSB) SET-ASIDE. This requirement is being issued as Request for Proposals (RFP) No.VA-263-11-RP-0659. The FAR clauses and provisions enclosed are those in effect through Federal Acquisition Circular (FAC) 2005-53 The Department of Veterans Affairs, VA Midwest Health Care System (VISN 23) Network Contract Activity (NCA) on behalf of the Sioux Falls Health Care System intends to award a single, firm fixed-price contract for completion of construction project 438-11-119 "Construct Fitness center, Sioux Falls VA Health Care System ", at the Sioux Falls VAMC, Sioux Falls, South Dakota. The applicable NAICS Code is 236220 with the applicable size standard being $33.5 million. The estimated price range is between $100,000.00 and $250,000.00. The period of performance is 75 days after issuance of the Notice to Proceed for construction which is expected to be issued in December 2011. A firm, fixed price contract will be awarded pursuant to FAR parts 15 and 36. Award will be based on determination of best value to the government and may not be to the lowest price. Work is to be completed in accordance with the project drawings and specifications. Work includes, but is not limited to, providing all labor, materials, tools and equipment and other resources required to construct a new physical fitness facility. Primary work for this project includes wall demo, wall construction, plumbing, wiring, HVAC and painting. In accordance with FAR 52.219-14(b)(3), General Construction, the Contractor must perform at least 15% of the cost of the contract, not including cost of materials, with its own employees.
