- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and the commercial item test program of FAR 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This combined synopsis/solicitation is issued as a request for quote (RFQ) # F4D4A11210A002. Submit written quotes- quote template provided upon request- via email. Oral offers will not be accepted. All firms or individuals responding must be registered with the Central Contractor Registration (CCR) (www.ccr.gov). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-53 and DFARS Change Notice 20110819. This procurement is being issued as a 100% small business set aside under North American Industrial Classification Standard (NAICS) 621512 and Size Standard $13.5 million. Project Number: F4D4A11210A002 Project Title: Mobile MRI 1. Quote shall conform to the CLIN structure as established in this synopsis/solicitation. 2. Quotes shall be valid for 30 days after close of solicitation. 3. Quotes shall be submitted priced per month, with a grand total amount for all 12 months. 4. Please see attached Statement of Work for detailed description of required duties. 5. Performance shall be at Vandenberg Air Force Base, CA, shipping is Free-on-Board(FOB) Destination 6. Period of performance: Base Year: 30 Sep 11- 29 Sep 12 Option Year 1: 30 Sep 12-29 Sep 13 7. All questions or comments must be provided to the Contract Specialist in writing; telephone and other means of oral communication will not be permitted. The Contract Specialist is John P. Haring, john.haring@vandenberg.af.mil. The contract will be Firm Fixed Price with the following structure: Include a complete breakdown of all costs: CLIN 0001 Mobile MRI Service Base Year-12 Months CLIN 1001 Mobile MRI Service Option Year 1-12 Months The following Clauses apply to this award: FAR 52.204-9: Personal Identity Verification of Contractor Personnel FAR 52.212-1: Instructions to Offerors -- Commercial Items. FAR 52.212-2: Evaluation -- Commercial Items. The following factors shall be used to evaluate offers: Price and personnel. Price and personnel will be equal determinants in the award of the contract. Personnell will be evaluated on a pass/fail basis in regards to meeting the position requirements in the statemen of work.. Offerors shall submit resumes for personnel for the positions of technologist and coordinator stated in the proposal. Failure to submit resumes will result in a quote being considered nonresponsive.
