- Appendix C to PWS. Safety, Fire Protection and Health Specification, and Industrial Safety Requirements Performance Work Statement for Crushed Gravel WD 05-2531 rev. 11 dated 6/13/2011 This is a combined synopsis and solicitation for crushing gravel at Utah Test and Training Range (UTTR). This effort will be awarded as a FFP contract whose offer conforms to the requirements of the PWS and presents the lowest price. Period of performance is 90 business days after reciept of award. Item 1: Crushing Gravel at Utah Test and Training Range (UTTR) IAW PWS This synopsis and solicitation is prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and therefore quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote (RFQ) F2DTAG1236A001. Submit only written offers; oral offers will not be accepted. All firms or individuals responding must be registered with the Central Contractor Registration (CCR) and Online Representations and Certifications Applications (ORCA) http://www.bpn.gov/. This procurement is being issued as 100% SMALL BUSINESS SET-ASIDE for North American Industrial Classification Standard (NAICS) 212321 and business size standard not to exceed 500 employees.
