- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W912EQ-11-Q-0051 and is being issued as a Request for Quote (RFQ). This procurement is 100% Small Business Set-Aside. The associated NAICS code is 423320 with a size standard of 100 personnel. All responsible sources may submit a quotation, which shall be considered by the agency. Quotes may be submitted in hard copy or electronically, either through regular mail, email, or by facsimile. Regardless of method of delivery, it is the offeror's responsibility to ensure receipt of quotes by the time and date stated herein. Contractors shall submit a lump sum quotation on company letterhead to include all charges for the hauling of debris as stated in the PWS below: Performance Work Statement CLIN 0001: Provide 4,500 Tons of Clay Dirt Material -NTE Contractor shall provide all necessary labor, equipment, material, supervision, transportation and any services to provide, load and haul clay dirt material to Orleans Parish on Lake Pontchartrain Levee near Ruby Street. The burrow source must be a Corps of Engineers approved pit. The following description of material requirements is provided below: Requirements: Unit of MeasureQuantityDescription Tons4,500Approved Dirt and delivery to above locations. All quotes must have the approved pit information before quote is accepted. The quote shall also contain a 5-pt proctor results within the last 120 days. All quotes should be provided utilizing dump trucks and not fifth wheel dump bed trailers due to the limited maneuverability space. Dump truck shall be able to haul a minimum of 20 tons of dirt per trip. Contractor is required to provide drivers with a ticket that will contain the following information: a.Truck Identifier (License Plate number) b.Weight empty c.Weight loaded d.Tonnage hauled The 4,500 ton of clay dirt material to be hauled on this requirement is an estimated quantity. The maximum amount of clay dirt material to be provided under the resulting contract for this Request for Quote is 4,500 tons. The Contractor shall adhere to all OSHA standards and the Corps Safety Manual, EM 385-1-1. The period of performance is 90 days. Performance period will begin on the day the first load of material is hauled, which shall be within five days of contract award. Contractor shall submit completed clause 52.212-3, Offeror Representation and Certifications, Commercial Items. The following clauses and provisions are incorporated and will remain in full force in any resultant award.
