- Amendment 0001 to Solicitation W91YTV-11-T-0341 Questions and Answers: 1. Question: We offer our software both in an Enterprise (customer-hosted), or ASP (vendor-hosted, software as a service) offering. Does Fox Army Health Center have a preference for which of these options we should include in the quote, or should we provide pricing for both options? Answer: Provide price both ways. 2. Question: The solicitation mentions that we are to include a copy of the provisions at FAR 52.212-3, Offeror Representations and Certifications Commercial Items (AUG 2011) and Alternate I (APR 2011) with our quote. Unfortunately, the website listed in the solicitation is not responding. Are you able to provide a copy of these provisions in another form? Answer: No, we cannot provide a copy. Website - http://farsite.hill.af.mil/vffara.htm 3. Question: The solicitation requests that we submit one copy of the technical quote, including software description and integration services, and one copy of the price quote containing the completed CLINs table. Is there any additional information you require/request as part of this submission (e.g. resumes of personnel, references, etc.)? Answer: Yes resumes and references of the staff completing the integration is requested. Include any other DoD Medical Treatment Facility where the individual and/or company has provided services. RESPONSE DATE REMAINS: September 19, 2011, 1100 EST
