- Description
- The Department of Veteran Affairs, Medical Center, Fayetteville, Arkansas, has a requirement for the installation of two items of medical equipment in two different locations. The resulting Contract will be a Firm-Fixed Price Agreement. The Solicitation is issued as a Small Business Set-Aside. The Request for Quote (RFQ) is issued on 9/9/2011 with responses due on 9/14/2011, 11:00 am Eastern Standard Time. Interested parties responding to this Solicitation shall provide an e-mail response by the due date indicated on the Solicitation. Please e-mail your response to paul.gertiser@va.gov B.1 Contract Administration Data CONTRACTOR REMITTANCE ADDRESS: Effective January 1, 1999 all payments by the Government to the contractor will be made in accordance with Federal Acquisition Regulation (FAR) Clause 52.232-34, Payment by Electronic Funds Transfer. Contractor must register in the Central Contractor Registration (CCR) database in accordance with FAR Clause 52.212-1(k). E.1 Statement of Work: Equipment Installation for the Eyecap with BX900 Slit Lamp Package. Provide Total Labor Material, Equipment, Travel, and Supervision and Technical Knowledge to perform the installation to include the assembly and testing of the system, general care/maintenance and cleaning. The installation will be accomplished in two different Community Based Outpatient Clinics, Branson, Missouri and Mount Vernon, Missouri. The equipment will be delivered to those locations for the installation.
