- Q - Laboratory Testing Anatomical - The VAMC, 1101 Veterans Drive, Lexington, KY 40502-1962 requests offers for providing anatomic and clinical referral laboratory testing. Tests will be conducted by offeror off-site. NAICS code of 621511 has a size standard of $13.5 million. This is conducted on an unrestricted basis. The provider must maintain College of American Pathologists accreditation for laboratory services. Services are required 24 hours a day seven days a week including holidays. Specimens shall be picked up and transported by contractor and delivered to VAMC Lexington, KY site at contractor expense. Solicitation shall be available on or about 9/9/2011 with due date of 9/16/2011 for base year and one-year option period. Evaluation factors are past performance, lab accreditation and lowest aggregate price.
