Description
Performance Work Statement This is a combined synopsis/solicitation for non-personal services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is F3Z4531193A001; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-53 effective 5 July 2011; Defense DPN 20110819 effective 19 August 2011, and AFAC 2011-0714 effective 14 July 2010. The North American Industry Classification System (NAICS) code is 611699. The business size standard is $7,000,000.00. The Federal Supply Class (FSC) is R499. The Standard Industrial Classification (SIC) is 8299. Travis AFB intends to award a purchase order for mind fitness training services for the Health and Wellness Center at the David Grant Medical Center. The requirement is unrestricted. CLIN 0001 - Local mind fitness training providing a curriculum for Airmen to learn how to recover quickly from stress. The training must address mind fitness in relation to combat readiness and must have a proven record of successful use in the military. The training must create resilience and must be made accessible and relevant with application to the real-world operational environment. Course materials and all trainer travel expenses must be included. The training shall meet all the requirements specified in the Performance Work Statement. (b) The use in this solicitation or contract of any Defense Federal Acquisition Regulation (48 CFR Chapter 2) clause with an authorized deviation is indicated by the addition of "(DEVIATION)" after the name of the regulation. 252.204-7006 BILLING INSTRUCTIONS (OCT 2005) 252.212-7001 CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT STATUTES OR EXECUTIVE ORDERS APPLICABLE TO DEFENSE ACQUISITIONS OF COMMERCIAL ITEMS (DEC 2010) (DEVIATION) 5352.201-9101 OMBUDSMAN (10 AUG 2005) (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMBC A-76 competition performance decisions). (d) The ombudsman has no authority to render a decision that binds the agency.
