Description
THIS IS A SOURCES SOUGHT ONLY. This is not a solicitation announcement. No contract will be awarded from this announcement. No reimbursement will be made for any costs associated with providing information in response to this announcement. The Architect of the Capitol is seeking physicians that may be capable of reviewing the tests performed by the Office of the Attending Physician personnel and conducting physical examinations of the employees. It is anticipated that certain evaluations will need to be conducted in the clinical setting of the contractor's facilities. Therefore the contractor must have a fixed location convenient to the Metropolitan Washington, D.C. area. The contractor must also have X-ray equipment that will provide film suitable for evaluation by a NIOSH certified A B @ reader. It is anticipated that in the near future NIOSH will recognize digital radiography for use in the surveillance of workers exposed to asbestos. When that occurs, the OAP will perform all X-rays. The contractor will provide the necessary evaluation of the X-ray. The contractor shall provide information describing the ability of their fixed location to provide the required physical examinations. The contractor may need to provide consultative services on site to address concerns in the area of Occupational and Environmental Medicine. The Contractor shall: a. Provide chest X-rays when required. b. Have the x-ray film read by a NIOSH certified A B @ Reader. c. Review the results of spirometry and audiometry testing. d. Interpret the results of electrocardiograms. e. Provide all materials needed for the collection of specimens and shall be responsible for the coordination and delivery of all laboratory test samples which must be sent out for analysis, and must make arrangement for the same. f. Provide interpretation of the results of all laboratory analysis. For the services which are to be provided at the Capitol Complex an Occupational Health Nurse will be available to assist in the management of physical examinations. The AOC shall provide the Contractor with the work schedule and description of duties of all employees who shall receive a physical examination. The OAP shall coordinate with AOC personnel in scheduling employee physical examinations pursuant to the employee's work schedule, and shall assist the AOC in the planning, organizing, and completion of physical examinations, as requested. The examination site is anticipated to be the First Aid Unit within the Rayburn House Office Building. For services which may need to be provided at the contractor's location, the Contractor shall provide all specialists and materials necessary to complete physical examinations, to include any and all clinical testing, specimen collection, X-Rays, etc. required to perform this task. At least four weeks in advance of site work, the Contractor shall provide the following information: a) Names and Social Security Numbers on the Contractor's letterhead, of all Contractor personnel to be working on-site. b) A description of the transportation needs for equipment and material. Any vehicles making deliveries at the U.S. Capitol must comply with the requirements of the U.S. Capitol Police. A copy of these requirements is enclosed. The physical examination shall be performed only by a licensed physician who shall be Board Certified in Occupational Medicine. All other medical testing, as outlined in Appendix A, shall be performed only by those trained and certified in each procedure, and shall be under the direct supervision of a licensed physician who shall be Board Certified in Occupational Medicine. If necessary, completed tests may be sent to another location for reading and analysis. At least four weeks prior to the start date of physical examinations, the Contractor shall provide the Office of the Attending Physician, Environmental Health Division with pre-physical questionnaires. The questionnaires for the Asbestos protocol shall be in the OSHA approved format. Questionnaires for other protocols shall be reviewed and modified if deemed necessary. Questionnaires shall be filled out, signed, and brought by the employee to the scheduled medical physical. The Contractor shall also have available on site, enough "medical history" forms for completion, equal to the number of individual medical protocols anticipated for completion. On the day of the examination, the physician performing the physical examination shall issue fitness for duty clearances to those employees who are physically qualified to wear respirators. RESULTS OF EMPLOYEE PHYSICAL EXAMINATIONS: Within four weeks after the completion of the physician examinations, the contractor shall provide the following reports to the Office of the Attending Physician, Occupational Health Division: a) The physical examination form. b) Spirometry results (in liters and % predicted) c) X-ray results with the B-reading when done. d) All laboratory value results. e) Copies of all information or recommendations which are provided to employees. f) Copies of any and all Physician's written opinions including: 1. All those written opinions specified by OSHA regulations. 2. A statement on the medical fitness or qualification of an employee to perform a specific job or to work with a specific toxicant. 3. A statement as to any evidence that an employee has a health condition related to previous exposures from an exposure agent. 4. A statement on any other medical conditions which may be unrelated to workplace exposures, but which will require medical follow-up or treatment. g) A written statement which is suitable for providing to the employees supervisor stating the employees fitness or qualification to perform a specific job or to work with a toxicant. h) The Contractor and his agents shall, for any time period after completion of medical surveillance physicals for the AOC, be required to respond in writing to questions by the AOC, concerning the health of an employee, based upon the hands-on physical examination by the Physician, Board Certified in Occupational Medicine. EMPLOYEE RESULTS : Within 45 days of the medical examination, each employee receiving a medical physical shall receive an individual medical report of same at a location designated by him/her. An individual report shall be provided for each physical given. Failure to comply with this requirement shall result in delay of payment until all examinees have received medical physical results. In the event that repeated attempts of delivery to employees are unsuccessful, the Contractor must forward said employee results to the Office of the Attending Physician, Environmental Health Division, B-344 Rayburn House Office Building, Washington, D.C. 20515 and shall be prominently marked " EMPLOYEE RESULTS PERSONAL & CONFIDENTIAL @ ATTN: Occupational Health Nurse. All test results shall be treated as confidential medical records. All other requirements as set forth by government regulation for records retention shall be met. The Office of the Attending Physician, Occupational Health Division shall maintain medical records. Due to limited space, parking of the Contractor Staff on properties under the jurisdiction of the AOC may not be possible, and we strongly encourage the use of public transportation, (i.e. METRO Transit System, Capitol South Station). However, it may be possible to arrange parking for vehicles carrying equipment. Field discretion by representatives of the Architect of the Capitol for deviation from scheduled medical testing shall be allowable, as time permits, based upon proof and or reasonable suspicion that an exposure exists, which justifies additional medical testing. This decision can be made on-site, by the Occupational Health Nurse representing the Architect of the Capitol, and must be approved by this representative before completion by the Contractor. Medical Testing Protocols The AOC has identified ten health screening protocols for testing. Where no health screening protocol exists, recommendations are based upon generally accepted medical practice. The following protocols are the absolute minimum health screening requirements which shall be provided by the Contractor. The Contractor shall be allowed to make recommendations on supplemental medical testing, based upon reasonable and prudent medical clinical judgment. Medical Testing Protocol should be referenced with Appendix A. The following health screening protocol shall be utilized by the Contractor providing services: 1) Asbestos - Health screening protocol shall include work and medical history (on the OSHA periodic form), Chest X-Ray, (when indicated) Spirometry, and Physical examination to include vital signs. This protocol shall be completed in compliance with all requirements of 29CFR1910.1001. All employees in this category shall be evaluated for the use of a respirator. 2) Suspected Past Exposure to Asbestos - Health screening protocol shall include: work and medical history (on the OSHA periodic form), Chest X-Ray (when indicated), Spirometry, and a Physical examination to include vital signs. This protocol shall be completed with reference to the requirements of 29CFR1910.1001. 3) Methylene Chloride - Health screening protocol shall include: work and medical history; spirometry; electrocardiogram; physical examination of the skin and appendages, head, eyes, chest, heart, abdomen, and nervous system; laboratory work including hemoglobin and hematocrit, alanine aminotransferase (ALT, SGPT), and when possible post-shift venous blood carboxyhemoglobin. This protocol shall be completed with reference to the requirements of 29CFR1910.1052 App B. 4) Crystalline Silica - Health screening protocol shall include: medical history, work history describing silica exposure, physical examination (with attention paid to potential scarring of the cornea), a chest roentgenogram (x-ray) and spirometry. This protocol shall be completed with reference to the requirements of OSHA Directive CPL 2-2.7. 5) Organophosphate/Carbamate Pesticides - Health screening protocol shall include work and medical history, RBC, and serum cholinesterase level analysis. 6) Lead - Health screening protocol shall include work and medical history, baseline blood lead and ZPP levels, Complete Blood Count with differential, and physical when indicated. This examination/protocol shall meet allrequirements of 29CFR1910.1025. 7) Respirator- Health screening protocols shall be based upon the type of respiratory protectionbeing used, and the degree of work activity. Use of disposable respirators shall require only work and medical history, spirometry, and vital signs. Use of negative pressure, SCBA, full half-mask respirators, and those involved in moderate to heavy work assignments shall need a physical examination. The contractor will be provided with the following information: Type of respirator to be used; Frequency of use; Duration of use; Work intensity while respirator is used; Expected exposures and level of exposure. Medical qualification/clearance shall meet all the requirements of 29CFR1910.134. 8) Noise- Health screening protocols shall include work and medical history, and audiogram. 9) Heat- Health screening protocols shall include work and medical history, and the recommendations of the NIOSH Occupational Exposure to Hot Environments (Revised Criteria 1986) Clinical Testing Specifications The Contractor shall comply with the following clinical testing specifications, and provide documentation of compliance if requested: 1) Audiometry- Audiometric Testing shall be performed by Certified hearing Conservationists. A certified booth shall be provided and clinical testing procedures shall meet both the OSHA Standard 29 CFR 1910.95, as well as the ANSI Standard 83.6-1969. The frequencies to be tested shall be 500, 1000, 2000, 3000, 4000, 6000, and 8000 hertz. Test results shall be interpreted by a Physician, Board Certified in Occupational Medicine, and recommendations for further evaluation will be made, if indicated. 2) Blood Pressure- Blood pressure shall be recorded for each employee by technicians who are trained in the utilization of hospital grade sphygmomanometers and stethoscopes. Diagnosis of hypertension shall be done in accordance with Joint National Committee on Detection, Evaluation, and Treatment of high blood pressure: The fifth report, Archives of Internal Medicine 153: 154, 1993. Patients will be immediately notified of any significant abnormality. 3) Spirometry - Examinations shall be performed by technicians having passed NIOSH approved curriculum courses in occupational spirometry. The analytical equipment and testing technique shall meet the current American Thoracic Society standards as written in Standardization of Spirometry, 1994 Update, American Journal of respiratory and Critical Care Medicine, Vol. 153 pp1107-1136, 1995 with appropriate correction for temperature and pressure. Results for per cent of predicted normal values should be interpreted in accordance with 29CFR1910.1043 (Cotton Dust Standard). The required tests shall be FVC, FEV1, and FEV1/FVC, and percent of predicted normal estimate. 4) Chest X-Ray- A Chest X-Ray shall be administered by a registered and licensed ARRT Technologist. Those Chest X-Rays done for the asbestos protocol or other agent known to cause pneumoconiosis shall be interpreted by a NIOSH certified "B" reader. 5) Electrocardiograms- Twelve lead Electrocardiograms (EKG) shall be performed, only by trained technicians, and interpreted by a physician skilled in the Interpretation of electrocardiograms. This report shall contain appropriate commentary regarding the EKG, with follow-up recommendations when medically prudent. 6) Physical Examination- Physical examinations shall be conducted by licensed Physicians, who shall be Board Certified in Occupational Medicine. Examinations shall begin with a review of the medical history as submitted by the patient. It shall also include the following protocol: recordings of official height and weight, examination of eyes, ears, nose, and throat, thyroid, skin, lymph system, heart and lungs, thorax, abdomen, extremities, spine, and review of the musculoskeletal system, blood pressure, pulse, and respiratory rate. The physician shall also review all tests and laboratory studies required by the protocol or done at the discretion of and upon the request of the examining physician. Patients shall be advised of abnormalities identified during the exam while on-site, and shall be recommended for immediate follow-up with their own personal physician. 7) Laboratory Analysis- Specimen collection shall be performed by trained phlebotomists, to insure proper procedures for accurate sampling are observed. Evaluations of blood lead shall be performed by a CDC certified laboratory. All blood sampling shall be obtained by closed Vacutainer technique, with strict adherence to Universal Precautions and OSHA Blood borne Pathogen standards. Specimen handling and medical waste disposal shall also comply with all applicable regulations, with documentation upon request. The Contractor shall be responsible or assuring that all laboratory samples are received by the laboratory for analysis. 8) Biohazardous Medical Waste - All biohazardous medical waste accrued on site, as a result medical examinations conducted for the AOC shall remain the responsibility of the contractor, who upon request shall be required to produce certified copies of the biohazardous waste disposal manifest. DOCUMENTATION The Offeror must submit: a) Written Medical quality control and assurance plans b) Written Blood borne Pathogens Policy. •c) Physician Qualifications 1. Curriculum Vitae and documentation that the proposed examining physician is board certified in Occupational Medicine. 2. Documentation that a certified B-reader shall provide interpretation of all the chest x-rays done for the evaluation of pneumoconiosis (asbestosis, silicosis). d) Laboratory Certifications. e) A minimum of two (2) similar clients for whom examinations were provided within the last two (2) years, with contacts. METRO ACCESSIBLE : Facilities and services MUST be convenient to Metro. INSURANCE : An A ccord Insurance Summary Form, detailing Insurance Carrier, policy numbers, etc. including Limits of Liability must be submitted within 10 days of contract award. Contractors with the capability of providing services described above are invited to submit, in writing, sufficient information which demonstrates their ability to fulfill the requirements. Capability Statements shall be limited to no more than three (3) pages. Standard brochures will not be considered a sufficient response to this notice. Capability Statements should include the following: (1) Name of vendor; (2) DUNS Number; (3) Business Size; (4) Contact Information; (5) Description of the capabilities possessed for meeting this requirement. Contractors should submit an electronic copy of the requested information to Carmelita E. Wesley, at cwesle@aoc.gov no later than 6:00 P.M., EST, September 6, 2011. No feedback or evaluations will be provided regarding the information provided. All respondents must be registered in the Central Contractor's Registration (CCR) file, which may be accomplished via the Internet at http://www.ccr.gov.
