- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-53, Effective 04 Aug 2011 and Defense Federal Regulation Supplement (DFARs), current to DPN 20110819 (Effective 19 August 2011) Edition. It is the responsibility of the contractor to be familiar with the applicable provisions and clauses which can be accessed in full text at www.farsite.hill.af.mil. This solicitation is being issued as a Request for Quotations (RFQ) under Solicitation Number W81R8T1230RM01. This action is being solicited as a 100% small business set-aside. The North American Industry Classification System (NAICS) code is 332311 with a size standard of 500 employees. Quotes are being solicited on a Lowest Price, Technically Acceptable (LPTA) basis. All quotes shall include transportation costs to U.S. Army Yuma Proving Ground (USAYPG), Yuma, Arizona 85365-9498. The USAYPG, Yuma, Arizona is requesting quotes for a total of 7 Large Connex Boxes. Please see Attachment 2 - Connex Box Specifications for sizes and required specifications. Offerors must provide the Unit Price, Total Price, and Total Extended Price. No partial bids will be accepted. Please annotate on your quote whether you would prefer VISA or WAWF payment. Offerors shall account for any costs associated with accepting credit card payment. Pursuant to FAR 52.212-5, the Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government. This RFQ closes on 08 September 2011 at 12:00 Noon Mountain Standard Time (MST). Responses to the RFQ shall be submitted by e-mail to jason.w.house@us.army.mil by the closing date indicated in this CSS. Please reference RFQ W81R8T1230RM01 on all inquiries. All responding vendors must be actively registered with the Central Contractor Registration (CCR) and the Online Representations and Certifications Application (ORCA) prior to award of contract. Information to register in CCR can be found at http://www.ccr.gov/(https://www.bpn.gov/ccr/default.aspx) and ORCA at https://orca.bpn.gov/. As a minimum, offerors shall include with their signed quotation the following information: (1) Company name and address; (2) Point of contact with telephone and facsimile numbers and email address; (3) A completed copy of FAR 52.212-3, Representations and Certifications with Alternate I; or if you have completed the annual representations and certifications at the Online Representations and Certifications Application (ORCA) website, you are only required to submit a signed copy of FAR Clause 52.212-3(b); (4) Proposed price to include any applicable freight. Arizona vendors are to include the Arizona Transaction Privilege Tax, if applicable; (5) Proposed delivery schedule; (6) A statement indicating that the Government Visa Credit Card will be accepted as the method of payment and/or a preference for WAWF payment; and (7) Acknowledgment of Solicitation Amendments. Offerors that fail to furnish required representation or technical information as required by FAR provisions 52.212-1 or reject the terms and conditions of this solicitation may be excluded from consideration.
