Description

This a pre-solicitation notice for halfway house services in accordance with FAR Part 5. The Department of Veterans Affairs, Tuscaloosa Veterans Affairs Medical Center (TVAMC), Tuscaloosa, AL is seeking sources for a potential contract to provide Transitional Housing for Homeless Veterans Services at the contractor facility for authorized beneficiaries of TVAMC. The contractor's facility must be within the TVAMC catchment and within 0.5 miles of public transportation. The resulting contract will be for a total of 5 years if all options are exercised (which are at the Government's discretion). The IDIQ type contract is for halfway house services as specified in the schedule, performance work statement, and as outlined in the attachments thereto. This contract will be for a one year base period with the Government having the option to exercise four (4) one-year option periods in addition. Note: this requirement mandates that a contractor be in compliance with the Life/Safety checklist at the time of the submission of a proposal or the proposal will be considered non responsive. Contractor services will be furnished to beneficiaries for whom such care is specifically authorized by the Department of Veterans Affairs (VA) and are homeless with substance abuse and/or other mental health diagnosis. Negative drug screens will not be a requirement for admission. It is understood that the type of patients to be cared for under this contract will require care and services above the level of room and board. The per diem rate established will be an all inclusive rate. Care will include, but not be limited to: 1.Lodging for ten (10) homeless veterans 2.Structured group activities, including physical activities as appropriate. 3.Instruction in and assistance with health and personal hygiene. 4.Monitoring of medications, including locked drawers for storage of medication. 5.Supportive social services, in collaboration with the HCHV/HUD-VASH program staff, or other contract resources. 6.Individual case management, including professional counseling on self care skills, adaptive coping skills as appropriate, vocational rehabilitation referrals. 7.Assistance in learning and development of responsible living patterns to achieve a more adaptive level of psychosocial functioning upgraded social skills and improved personal relationships. 8.Support for an alcohol/drug abuse free lifestyle by maintaining a drug and alcohol free environment. 9.Routine and as needed/indicated breathalyzer and urine drug screening. 10.Assistance in learning, testing and internalizing knowledge of the illness/recovery process for homelessness. 11.Employment placement and job training. 12.Housing services to include permanent affordable placement. 13.No pornographic material allowed on the premises. This applies to staff and Veterans. 14.Employees should not fraternize with Veterans. The anticipated award date: 30 September 2011.
