PDF - Sources Sought SOURCES SOUGHT FOR CLINICAL RESEARCH SUPPORT The 802 Contracting Squadron is seeking sources capable of providing Diabetes Center of Excellence (DCOE) support to San Antonio, Texas Department of Defense Military Treatment Facilities (MTF). This notice is for planning purposes only and shall not be construed as a formal solicitation or an obligation on the part of the Government to acquire any products or services. PROJECTED START DATE IS 01 JAN 12. The Government is not liable for any costs incurred by interested parties in responding to this request for information. The purpose of this request for information is to determine the capability of potential sources and the availability of commercial sources pursuant to FAR Part 10. Industry is encouraged to respond with information not constrained by proprietary data rights. However, if proprietary data is included in your reply, please mark it appropriately. This is open to both large and small businesses. The following is a brief synopsis of services but not all inclusive. 1. Description: Contractor shall provide Data Analyst services to include but not limited to: Ensures the validity of clinical trials and formats them for statistical purposes. Designs collection instruments, sets up databases, and tracks/manages the flow of data to and from the investigative sites. Reviews data discrepancy resolutions provided by the investigative sites and enters corrections in the database, as appropriate. Assists in the review of interim/final data listings prior to transmission to other groups or inclusion in interim/final reports. Contractor shall provide Research Coordinator services to include but not limited to: Participates in the design, implementation and monitoring of clinical trials, preparation of integrated medical reports, etc. Assists in planning and implements all activities required to conduct and monitor complex trials and ensures that Good Clinical Practices (GCPs) are followed. Enrolls/performs informed consent of patients entering research trials based on research protocols. Assists principle investigator with the preparation of manuscripts for technical journals and makes presentations at scientific meetings. Assists in planning and execution of all aspects of the Diabetes Center of Excellence in concert with Air Force Diabetes Program strategic plan, goals and objectives. Contractor shall provide Medical Support Assistant services to include but not limited to: Records, compiles, transcribes, distributes meeting minutes. Assists with office services, such as record keeping, monitoring budget/expenses, personnel records, and housekeeping. Prepares clinical reports, memos, letters, and other documents, using word processing, spreadsheet, database, and/or presentation software. Prepare responses to correspondence containing routine inquiries. Contractor shall provide Disease Management Program Administrator services to include but not limited to: Develops and participates in outreach activities designed to improve diabetes care and prevention at external MTFs. Develops and participates in external training and marketing of DCOE outreach initiatives. Works with DCOE leadership in support of Air Force Medical Service initiatives for diabetes management (e.g., e-consult initiative, Project ECHO, Virtual Medical Clinic, Diabetes Champion Certification). Leads consultant team to external MTFs for assistance in developing a diabetes disease management strategy for their diabetes population for implementing diabetes prevention programs and provides continuing support for developed programs. Provides direct care to patients in the Diabetes Center of Excellence in order to maintain currency and relevance to the outreach mission. Contractor shall provide Diabetes Educator services to include but not limited to: Provides health education expertise, consultancy, and advocacy to support a comprehensive Air Force Diabetes Program. Provides individuals, families and groups on a variety of health-related topics employing age-specific and behavioral learning theory to include training patients and families on how to test blood. Participates in teleconferences, updates, working groups, review panels, symposiums, etc. related to diabetes education and management. Contractor shall provide Dietitian services to include but not limited to: Provides nutrition care to diabetic patients or patients determined to be at risk for developing diabetes. Maintains data, prepares reports and presentations as needed in support of the Diabetes Program. Administers nutrition assessment, prescriptions and counseling in accordance with diabetes program. Conducts nutrition screening (either manually or automated) of patients upon enrollment into the Pediatric Wellness Clinic or into a diabetic protocol study to determine nutrition risk and level of care required; collect nutritionally relevant data, e.g., medication, clinical symptoms, laboratory values, dietary history, anthropometrics, etc., from the medical record consistent with screening and assessment guidelines for all age specific groups; and communicate findings to the clinical director. Contractor shall provide Exercise Physiologist/Preventionist services to include but not limited to: Provides consultation and workshops with family members, schools, hospital staff and other personnel that work with diabetic and pre-diabetic patients. Provide outreach services to additional military sites. Organizes and counsels fitness, health, and prevention classes as outlined by diabetes program for the 12 Week Diabetes Prevention Class. Assists in planning and execution of all aspects of the Diabetes Center of Excellence in concert with Air Force Diabetes Program strategic plan, goals and objectives. Contractor shall provide Nurse Practitioner services to include but not limited to: Treats patients with diabetes acute and chronic conditions within accepted protocols, Nurse Practice Acts, and/or in collaboration with a physician. Requests consultation or referral with appropriate physicians, clinics, or other health resources as indicated. Prescribes and dispenses medications as delineated by the Pharmacy and Therapeutics Committee. Prepares reports, presentations and other written material in support of the Diabetes Program. Assists in planning and execution of all aspects of the DCOE in concert with Air Force Diabetes Program strategic plan, goals and objectives. Communicates and collaborates with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family. Implement/execute adult/pediatric/adolescent Diabetes Program requirements. Contractor shall provide Pediatric Nurse Practitioner services to include but not limited to: Performs medical assessments and treatments for the Pediatric/Adolescent Wellness Clinic population. Treats patients with weight related diabetes acute and chronic conditions within accepted protocols, Nurse Practice Acts, and/or in collaboration with a physician. Prescribes and dispenses medications as delineated by the Pharmacy and Therapeutics Committee. Prepares reports, presentations and other written material in support of the Diabetes Program. Communicates and collaborates with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family. Synthesizes data from a variety of sources and make appropriate clinical decisions. Contractor shall provide Pediatrician services to include but not limited to: Performs medical assessments and treatments for the Pediatric/Adolescent Wellness Clinic population. Population includes patient from 2 years to 23 years. Care shall include but are not be limited to routine general pediatric medicine care to include care for acute and chronic patient conditions, specifically diabetic and pre-diabetic children/adolescences. Administers patient care to include approving/disapproving subspecialty referrals, answering patient telephone consult with the assistance of clinic staff, refilling prescriptions, providing primary and secondary preventative maintenance care. Diagnose and treat diseases and injuries to include, but not limited to the following: obesity related conditions such as diabetes, hypertension, hyperlipidemia, PCOS, and metabolic syndrome. Maintains data prepares reports and presentations as necessary in support of the Diabetes Program Strategic Plan. Provides consultative services in support of the Pediatric Wellness Clinic and the Air Force Medical Service (AFMS) Medical Home Pediatric Obesity assessment and management program. Advises patients, parents or guardians and community members concerning diet, activity, hygiene, and disease prevention. Conducts research to study anatomy and develop or test medications, treatments, or procedures to prevent, or control disease or injury. Facilitates activities of nurses, students, assistants, specialists, therapists, and other medical staff. Contractor shall provide Research Program Administrator services to include but not limited to: Designs and writes of protocols, case report forms and informed consent forms for clinical trials. Participates in the design, implementation and monitoring of clinical trials, preparation of integrated medical reports, etc. Provides strategy on recruitment/selection on new investigators, contract research organizations and outside vendors. Enrolls/performs informed consent of patients entering research trials. Monitors and participates in the compilation/writing of integrated medical reports. Prepares manuscripts for technical journals and makes presentations at scientific meetings. Actively seeks research grant opportunities, and prepares grant proposals for research projects in accordance with Air Force Medical Service (AFMS) and DCOE research goals. Actively participates in the Air Force Diabetes and Obesity Research Working Group. Serves as the primary administrator for this group, to include establishing, documenting, and tracking administrative and research tasks in accordance with Working Group goals and timelines. Provides comprehensive coordination of the entire AFMS diabetes research program. Occasional travel required to additional military sites to provide education and consultation related to Diabetes Program outreach efforts. Contractor shall provide Training Program Administrator services to include but not limited to: Provides curriculum development of diabetes champion certification, and administrates this program. Provides shadowing opportunities within the DCOE for physicians, providers, nurses, and clinic support staff from other clinics/MTFs. Coordinates diabetes symposiums; independent or alongside other military public health forums. Provides care to patients in area of clinical expertise as needed to maintain competency and relevancy to the training mission. Contractor shall provide Clinic Nurse Manager services to include but not limited to: Assess outpatients' knowledge, skill and compliance with respect to recommended weight management behaviors. Maintain data (e.g. productivity reports) related to patient and staff education activities. Coordinates patient care through a continuum and facilitate the achievement of optimal outcomes in relation to care, quality and cost effectiveness. Ensures compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the medical treatment facility. Maintains data related to Wellness Clinic Program to include the AFMS Pediatric Home Obesity prevention and management program. Prepares reports, presentations and other written material in support of the Diabetes program. Contractor shall provide Behavior Counselor services to include but not limited to: Assesses patients and families to identify mental health and behavioral issues that impact their overweight, obesity, or diabetes and may interfere with their ability to participate in a behavioral management program. Counsels groups and individuals to assist them to identify behaviors and problems related to their weight-related conditions, and barriers to change. Assists in developing personalized recovery programs that help to establish healthy behaviors and lifestyle changes. Works as a part of the multidisciplinary team to develop a comprehensive treatment plan Contractor shall provide Medical Technician services to include but not limited to: Serves as a medical assistant performing care and assessments, establishes policies, procedures, protocols, and guidelines. Provide initial assessment to include vital signs, and other requirements as dictated by clinic policy. Performs foot exams in protocol of diabetic care. Provide training related to diabetic care, especially foot assessment and care, to staff from other DoD clinics and facilities. 2. Qualifications, Education, and Experience: (at a minimum and not all inclusive) Data Analyst: Bachelors of Science (BS) is required. Minimum of three (3) years experience of clinical data management experience is required within the last four (4) years. Research Coordinator: BS in a scientific or health care field from an accredited institution (e.g., nursing, pharmacy, physician's assistant) is required. Minimum of five (5) of relevant clinical research experience is required years within the last six (6) years. Medical Support Assistant: High School Graduate required. Minimum of two (2) years experience in health related area is required within the last three (3) years. Disease Management Program Administrator: BS degree in a health-related field from an accredited institution is required. RN-FNP, BC-ADM is preferred. Minimum of two (2) years experience in disease management experience is required within the last three (3) years. Diabetes Educator: Bachelor's degree in a health-related field from an accredited institution is required. Certified Diabetes Educator (CDE) eligible. Minimum of two (2) years age-specific experience as a diabetes educator is required within the last three (3) years. Dietitian: Registered Dietitian. Minimum of two (2) years experience in a health related field is required within the last three (3) years. Exercise Physiologist/Preventionist: Bachelor's degree in a health-related field from an accredited institution is required. Minimum of two (2) years experience in a health-related field is required within the last three (3) years. Nurse Practitioner: Master of Science in Nursing Degree required. Must have a minimum three (3) years in active clinical practice is required within the last four (4) years. Pediatric Nurse Practitioner: Master of Science in Nursing Degree required. Minimum of two (2) years experience in a pediatric related field is required within the last three (3) years. Pediatrician: Current board certification by the American Board of Pediatrics. Completion of a residency in Pediatric medicine acceptable to the Surgeon General, United States Air Force is mandatory. Minimum of two (2) years experience in a pediatric related field is required within the last three (3) years. Research Program Administrator: MD or PhD in a scientific or health care related field. Minimum of five (5) years experience in a health related field is required within the last six (6) years. Training Program Administrator: BS in a scientific or health care field. MSN and CDE preferred. Minimum of five (5) years experience in administration/curriculum development is required within the last six (6) years. Clinic Nurse Manager: Current State Licensure as a Registered Nurse. Minimum of two (2) years health related experience is required within the last three (3) years. Behavior Counselor: BS in behavioral sciences, Licensed Clinical Social Worker. At least (2) years experience in a health-related field is required within the last three (3) years. Medical Technician: AA or Bachelors of Science in a health related field. Certified medical technician desired. Minimum of one (1) year experience in a hospital or clinic is required within the last two (2) years. 3. Contractors are requested to provide the following information: 1.
