- All Interested Parties: The Federal Medical Center (FMC) Lexington, KY is soliciting quotations under Request for Quote (RFQ) P01091100053 for Outside Reference Lab Testing. Please read the RFQ carefully and include all information requested in your quote. Award date will be approximately October 1, 2011, with offers due by September 23, 2011, 2:00 PM EDST. This solicitation is unrestricted. Delivery will be made to the Federal Bureau of Prisons FMC Lexington 3301 Leestown Rd. Lexington, Ky 40511. The contract type is a firm-fixed price. The Government will award to the responsible offeror whose offer, conforming to the solicitation, is considered to be most advantageous to the Government, with past performance and price considered, prices being rated higher than past performance. Faith-Based and Community-Based Organizations have the right to submit offers/bids equally with other organizations for contracts for which they are eligible. The solicitation will be made available on or before September 2, 2011 and will be distributed solely through the General Services Administration Federal Business Opportunities website: http://www.fedbizopps.gov. The site provides downloading instructions. All future information about this acquisition, including solicitation amendments, will also be distributed solely through this website. Interested parties are responsible for monitoring this site to insure they have the most up-to-date information about this acquisition. Payment will be made using Electronic Funds Transfer (EFT). Each Offer is required to complete their representations and certifications via the on-line Representations and Certifications Applications (ORCA) at http://orca.bpn.gov. All potential offerors must be registered in the Central Contractor Registration (CCR) database at http://www.ccr.gov. All communications regarding this RFQ may be made in writing to Janet Toy, Supervisory Contract Specialist, via telephone at (859) 255-6812 ext. 5131, or via email to jtoy@bop.gov. Please read the instructions that are incorporated by reference in FAR provision 52.212-1, "Instructions to Offerors-Commercial Items", and the addendum to the provision. An original of your quote must be submitted no later than September 23, 2011 2:00 PM EST, via email to jtoy@bop.gov. Offers will not be accepted via fax, unless otherwise authorized by the Government Contracting Officer. The Contracting Officer is the only individual who can legally commit or obligate the Government to an expenditure of public funds should a contract result from this RFQ. All communication prior to award shall be directed to the Contracting Officer as stated above.
