- Request for Quote SF18 Statement of Work Clauses/Reps & Certs Synopsis: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is 21-11-341DTE237 and is issued as a Request for Quote (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-44. The associated North American Industrial Classification System (NAICS) code for this procurement is 337211 with a small business size standard of 500. This requirement is a SMALL BUSINESS SET-ASIDE and only qualified offerors may submit bids. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34. The full text of the referenced FAR clauses may be accessed electronically at http://www.acqnet.gov/far. All Contractors doing business with the Government must be registered with the Central Contractor Registry (CCR). No award can be made to a company not registered in CCR. For additional information and to register in CCR, please access the following website: http://www.ccr.gov. In order to register all firms must have a Dun and Bradstreet Number. A Dun and Bradstreet number may be obtained free of charge by contacting Dun and Bradstreet at http://www.dnb.com/us/duns_update/ or 800.234.3867. Offerors are encouraged to go on-line at www.bpn.gov to complete the representations and certifications (ORCA). The U. S. Coast Guard, Honolulu, HI has a requirement for the services and/or supplies as listed below. Please fax quotation and completed 52.212-3 Offeror Representations and Certifications-Commercial Items form to BSU Honolulu (pc), Attn: Nadyne Cano at(808)842-2834 or email to Nadyne.V.Cano@uscg.mil as soon as possible, but no later than 12:00 p.m. HST, September 2, 2011. The contractor shall provide the following equipment as follows: Contractor shall provide materials, delivery, labor, assembly, installation, equipment, supplies and supervision for new office cubicles to be located at the US Coast Guard MSST, 400 Sand Island Rd., Honolulu, HI 96819 PER ATTACHED STATEMENT OF WORK. Location offices include two offices and one Chief Mess.
