- Amendment 0004 to RFI# FA5205-11-J-MM01 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; price information is being requested and a written solicitation will not be issued. The solicitation (Request for Information) reference number is FA5205-11-J-MM01. The solicitation incorporates provisions and clauses as those in effect through Federal Acquisition Circular 05-38. This procurement is NOT a set-aside. The North American Industry Classification System code (NAICS) is 423690 - Other Electronic Parts and Equipment Merchant Wholesalers. Offerors are required to forward information and technical documentation by 11:30 a.m. 5 Aug 11 (Japan Standard Time). DESCRIPTION: Instsallation of RFID Weapons Tracking System for Misawa AB, Japan. RFI No FA5205-11-J-MM01. PROVISIONS/CLAUSES: The following FAR and DFARS provisions and clauses apply: 252.203-7000, 252.212-7001, 252.232-7003, 252.232-7010, 52.204-10, 52.209-6, 52.212-4, 52.212-5, 52.222-19, 52.222-21, 52.222-26, 52.222-36, 52.223-18, 52.225-13, 52.232-33 http://farsite.hill.af.mil. ADDITIONAL NOTES: Include your DUNS number, CAGE code and Federal TIN. To obtain or renew a DUNS number or CAGE code, visit www.ccr.gov. Ensure your company's Representations and Certifications are current and up to date for this acquisition. Visit the Online Representations and Certifications (ORCA) website (http://orca.bpn.gov) to update or register your ORCA record. DEADLINE: Information shall be submitted by email to Ms. Mariko Metoki, mariko.metoki.jp@misawa.af.mil. Information is due by 11:30 a.m. JST on 5 Aug 2011. Amendment 0001 issued to clarify location of the installation. See attached SOW for details. Information submitted in response to this RFI may be used as the basis for award upon availability of funds, and confirmation provided by vendor. There will be no RFQ after this RFI. Closing date remains unchaged, 11:30 am JST on 5 Aug 11. Amendment 0002 issued to incorporate evaluation factor and extend solicitation for one week. Information shall be submitted by 11:30 am 12 Aug 11 (JST). Amendment 0003 issued to change FOB point to Destination (our APO address), add maintenance option requirement, and disseminate the Government's response to questions from prospective offerors. Closing date remains unchanged, 11:30 am 12 Aug 11 (JST). Amendment 0004 issued to provide clarification on the maintenance options and add clause. Closing date remains unchanged, 11:30am 12 Aug 11 (JST).
