- CLAUSES - FAR 52.212-5 SOW - Solid Waste Collection CLAUSES - FAR 52.252-1 TECHNICAL EVALUATION QUESTIONS CLAUSES - FAR 52.212-3 PRICE SCHEDULE This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Aberdeen Area Indian Health Service (IHS) intends to award a firm fixed-price commercial item purchase order in response to Request for Quotation (RFQ) 2011-032. This is a Small Business Set-Aside request. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53. The associated North American Industry Classification System code is 562111 and the small business size standard is $12.5 million. PURPOSE OF CONTRACT: See attached Statement of Work. Location is at the Pine Ridge IHS Hospital, E. Highway 18, Pine Ridge, South Dakota 57770. PRICE SCHEDULE: Solid Waste Collection: the quoted pricing must be all inclusive (to include but not be limited to transportation, food, lodging, per diem, etc.). The contractor is responsible for all associated costs for solid waste collection. 1. Complete attached Bid Schedule. Attached are the Federal Acquisition Regulations (FAR) and the Health & Human Services Acquisition Regulation (HHSAR) clauses that are applicable including the following: FAR 52.212-1 - Instructions to Offerors - Commercial Items (June 2008): Quotes shall be submitted on company letterhead stationery, signed and dated; it shall include: 1. Solicitation number 2. August 25, 2011 at 11:00am CST 3. Name, address, telephone number of the offeror and email address of the contact person 4. Technical description of the item/service being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary; 5. Terms of any express warranty 6. Price and any discount terms, including price schedule 7. "Remit to" address, if different than mailing address 8. A completed copy of the representations and certifications at FAR 52.212-3 9. Acknowledgement of Solicitation Amendments 10. A statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded form consideration. FAR 52.212-2 - Evaluation - Commercial Items (January 1999): 1. Quality and Environmental Issues: Include procedures and methods for: garbage disposal, protection of affected adjacent areas, operating on occupied facilities, MSDS sheets for chemicals to be used for cleaning and insect/rodent control. 2. Narrative of Project: Provide a comprehensive statement of the scope and purpose of the project. Include a detailed description of the proposed work involved to accomplish the objectives of the statement of work. Include either by chart or narrative: reason for subcontracts; estimated time frames for equipment delivery; proposed collection schedule for each location; proposed receptacle maintenance schedule for cleaning and insect/rodent control. 3. EPA certified land fill: Provide the name, location and proof of EPA certification for the landfill to be used for disposal. The OST Landfill may be utilized as the disposal area for this requirement as long as the OST Landfill remains compliant with section 2 below. The contractor will be responsible for utilizing an alternate compliant landfill or disposal method if the OST Landfill becomes non-compliant during the course of this requirement. 4. Past Performance: Include at least 3 references of past performance on similar projects. Include contact person name and title, company, address, phone number, and brief description of service performed or product supplied. A signed, written reference on company letterhead is preferred. Written references should include contractor's quality, workmanship, timeliness, professionalism, and overall performance rating. 5. Total Price for Contract: The total price for contract shall be itemized and include: dumpster rental, dumpster maintenance, collection charges, disposal fees, TERO fees and/or tipping fees. FAR 52.212-5 - Contract Terms and conditions Required to Implement Statutes or Executive Orders - Commercial Items (August 2011): See attachment for full text BASIS OF AWARD: Quoters are advised that the Government will consider the technical evaluation factors and the price in determining the successful contractor. The technical evaluation is worth 70% of the score and the price is worth 30% with an over-all score of 100%. The Government will consider the following factors in determining the successful contractor that represents "Best Value" to the Government.
