- Care and Certification Addendum for Declaration of Federal Employment, PL 101-647 Solicitation package - Adult Neuropsychological Assessment Screening The Albuquerque Area, Division of Clinical Quality, Behavioral Health Program, have a requirement for Adult Neuropsychological Assessment Screening. This patient population suffer from head injuries and other brain traumas and without accurate assessment screening, treatment for these patients is difficult. The Government anticipates in awarding a single fixed price requirements contract for a base year with four option renewals. Renewal of option years will be based on the Government's existing needs, availability of funds and satisfactory evaluation of contractor performance. The attached solicitation package comprise all information and forms to be completed along with the Offeror's technical proposal.
