- Description
- This is a pre-solicitation notice for synopsis of "Nurse Triage Answering Services" for Davis-Monthan AFB (DMAFB), Arizona. This is a commercial service; therefore, FAR Part 12, Acquisition of Commercial Items, shall apply to this acquisition. The contractor shall provide all facilities, equipment, tools, transportation, personnel, labor, supervision, and management to perform a 1-800 telephone-based Nurse Triage Answering Service on a 24/7 basis for the 355th Medical Group, 4175 S. Alamo Ave, Medical Treatment Facility (MTF), Building 400, Davis-Monthan AFB, AZ. The Davis-Monthan Nurse Triage Answering Service will provide clinical assessment and appropriate level of care support services for personnel assigned to Davis-Monthan AFB, AZ. This service shall be provided in accordance with the Performance Work Statement (PWS). This acquisition will be solicited as a Request for Proposal (RFP) utilizing FAR Part 15, Contracting by Negotiation. A Firm Fixed Price contract will result from this solicitation. Award selection will be based on Lowest Price Technically Acceptable and Negotiation. Period of Performance will be for a base period with four (4) one-year option periods. This acquisition will be a 100% Set-Aside for small business concerns as described in FAR Subpart 19.5. The North American Industry Classification System Code is 561421; and, the small business size standard is $7.0 million. The RFP Number is FA4877-11-R-0016 and shall be issued on or about 15 August 2011, posted electronically on the Federal Business Opportunity website (http://www.FBO.gov). No paper copies will be issued; no telephone request will be honored and no bidder's mailing list will be maintained. Potential offerors are responsible for monitoring this site for the release of the solicitation package and any amendments that may follow. Any proposal submitted is strictly voluntary. The government will not pay for any proposals submitted in response to the RFP. To be eligible for contract award, offerors must be registered in the Central Contractor Registration (CCR). Register via the CCR website (http://www.ccr.gov) or by contacting CCR Customer Service Contact Information at 1-866-606-8220. Please Note: The subsequent award of this requirement shall not be held up by the government waiting for a firm to be properly registered. All responsible small business concerns may submit a proposal to the 355th Contracting Squadron.
