- Attachment 4 Attachment 3 Attachment 2 Attachment 1 This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. The award resulting from this combined synopsis and solicitation will be a Firm Fixed Price purchase order for the items listed. The solicitation number for this procurement is FA4830-11-Q-0097 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-53, Defense Acquisition Circular (DAC) 91-13 Correction and Air Force Acquisition Circular (AFAC) 2011-0714. This acquisition will be set aside for Small Businesses in accordance with FAR 19.502-2(a). The North American Industry Classification System (NAICS): 337110. Small Business Size Standard: 500 Employees Quotations must be submitted on the attached Pricing Schedule for the following CLINS. 0001 Resurface Countertops-Tower Cab Contractor will provide all labor and materials to Resurface Countertop, and cut holes to allow adequate fit of consoles in the Tower Cab. Remove unwanted material and debris from Moody Air Force Base (Please see drawings attached) 0002 Remove/Replace Cabinets/ Countertops 6th Floor Break Room Remove and Replace with solid oak hardwood cabinets. Contractor will also provide labor and materials to Replace countertop and cut holes to allow adequate fit of sink in 6th floor Break Room. Remove unwanted material and debris from Moody Air Force Base (Please see drawings attached of the 6th floor break room EVALUATION/AWARD: IAW FAR 52.212-2, Evaluation-Commercial Items, the Government will award a purchase order to the quoter whose price is judged to represent the best value to the government. Evaluation criteria are as follows Lowest Price Technically Acceptable. Technically acceptability will be based on the contractor meeting qualifications and performance specifications in accordance with the drawings as attached. The quote which conforms to the government's specifications and is lowest-priced will be deemed the "lowest price technically acceptable" quote. Technically acceptable means the contractor must meet the requirements specified by the drawings. Quoters MUST BE REGISTERED ON CENTRAL CONTRACTOR REGISTRATION (CCR). Information concerning CCR requirements may be viewed via the internet at http://www.ccr.gov or by calling the CCR Registration Centers at 1-888-227-2423. Place of Performance: Moody AFB, GA Provision at FAR 52.212-1, Instruction to Offerors Commercial Items applies to this acquisition. Quotation (RFQ) should be submitted and shall contain the following information: RFQ NUMBER; TIME SPECIFIED FOR RECEIPT OF QUOTES; NAME, ADDRESS; TELEPHONE NUMBER OF QUOTER; TERMS OF THE EXPRESSED WARRANTY; ANY DISCOUNT TERMS AND ACKNOWLEDGEMENT OF ALL SOLICITATION AMENDMENTS (if applicable). Signed quotes must indicate quantity, unit price and total amount for each item. By signing, the quoter agrees with all terms, conditions and provisions included in the solicitation. Quotes shall also contain all other documentation specified herein. PERIOD OF ACCEPTANCE OF QUOTERS: The quoter agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an amendment to the solicitation. LATE QUOTES: Quotations or modification of quotes received at the address specified for the receipt of quotes after the exact time specified for receipt of quotes WILL NOT be considered. All quoters must be registered on Central Contractor Registration (CCR) and online representation and certification application (ORCA) must be activated. ORCA registration for on line representation and certification requirements may be viewed via the internet at www.bpn.gov.
