- Prevention (CDC) intends to negotiate, on a sole source basis, under the authority of FAR 13.106-1(b)(1), with University of California (UCLA), Los Angeles, CA to develop epilepsy and health-related Quality of Life survey based on the 2011 California Health Interview Survey conducted by UCLA. The vendor will administer and develop epilepsy questions on a sufficiently large, representative survey of community-based adolescents (12-17 years of age) to: 1) obtain population-based estimates of the prevalence of epilepsy and burden of impaired quality of life in adolescents (12-17 years) with epilepsy; 2) obtain valid epilepsy-related data in adolescents by race/ethnicity (necessitating a large sample size); 3) identify health disparities and unmet needs associated with epilepsy in adolescents; and 4) identify the burden of impaired health-related quality of life in adolescents (12-17 years) by race/ethnicity, not previously reported by CDC. Since UCLA developed and conducted the original survey and has the results, they are the only known vendor that can provide this service. The delivery date shall be no later than February 1, 2012. The North American Industrial Classification System (NAICS) code for this procurement is 541990. This notice is issued for informational purposes only. All interested sources may submit a quote which shall be considered by the agency. The circumstances of the contract action deem only one source reasonably available. CDC is not requesting competitive proposals and no solicitation is available. Firms that believe they can provide the services required are encouraged to identify themselves and give written notification to the Contracting Officer within 15 days after publication of this notice. Determination not to compete this proposed action based on responses to this notice is solely within the discretion of the Government. Information received will be considered for the purpose of determining whether to conduct a future competitive procurement. Respondents will assume all financial costs for preparing a response to this notice. The U.S. Government assumes no financial obligation for the preparation and distribution of prepared responses.
