- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W912EQ-11-Q-0034 and is being issued as a Request for Quote (RFQ). This procurement is 100% Small Business Set-Aside. The associated NAICS code is 238910 with a size standard of $14.0M. All responsible sources may submit a quotation, which shall be considered by the agency. Quotes may be submitted in hard copy or electronically, either through regular mail, email, or by facsimile. Regardless of method of delivery, it is the offeror's responsibility to ensure receipt of quotes by the time and date stated herein. Contractors shall submit a lump sum quotation on company letterhead to include all charges for the hauling of debris as stated in the PWS below: Performance Work Statement CLIN 0001: 20, 000 Yards debris hauled - NTE Contractor shall provide all necessary labor, equipment, material, supervision, transportation and any services to perform construction debris,rock, dirt, asphalt, etc, pickup and removal from east side of Lake Pontchartrain Levee, LVP 104, (approximately one mile from Lakeshore and Franklin Ave) in New Orleans to an approved landfill for disposal. Debris is located in Orleans Parish on east side of Lake Pontchartrain Levee and shall be hauled by contractor and dumped at a landfill. Government personnel will load contractor trucks. Hauling shall be done utilizing dump trucks and/or fifth wheel dump bed trailers. Contractor is required to provide drivers with a ticket that will contain the following information for each delivery: Truck Identifier (License Plate number) Yardage (COE with contractor will measure trucks at start of hauling. All quotes must have the landfill where material will be dumped. COE must receive a copy of each load ticked from landfill. The Contractor shall adhere to all OSHA standards and the Corps Safety Manual, EM 385-1-1. Performance shall begin within 3 day of award an shall last 30 days from 1st day of hauling. Contractor shall submit completed clause 52.212-3, Offeror Representation and Certifications, Commercial Items. The following clauses and provisions are incorporated and will remain in full force in any resultant award.
