Statement of Work (SOW) Early Intervention & Transition Assistance for OEF/OIF Combat Veterans & Their Families Background (or History): Public Law 110-387, section 302 requires VA to conduct a three year pilot program, in coordination with the Vet Center program, to provide readjustment and transition assistance in helping combat veterans and families cope with the challenges associated with making the transition from military to civilian life. VA plans to implement this program as a natural extension of the Vet Center mission to promote access and early intervention through outreach and preventive education. The program will target rural and minority combat veterans in and around the Native American Indian Reservations in Northern Montana (Blackfeet, Rocky Boy, Fort Belknap and Flathead Indian Reservations)who are at greater risk due to geographical and cultural barriers to access. The primary objective of this program is to assist OEF/OIF veterans who served in a combat zone with the active military service, reserve and/or National Guard to overcome obstacles to care through provision of Veteran peer services that are culturally competent. Definitions: OEF/OIF - Operation Enduring Freedom/Operation Iraqi Freedom COTR - Contracting Officer's Technical Representative, Department of Veterans Affairs RCS - Readjustment Counseling Service, Department of Veterans Affairs (Vet Center Program) CO - Contracting Officer, Department of Veterans Affairs Primary Objective: Provide readjustment outreach services and transition assistance to combat veterans and their families in their community. Provide these services with Veteran peer provider(s) who is(are) culturally competent for the target population, Northern Montana Indian combat veterans It is projected that the employee(s) will be equivalent to the Federal General Grade Schedule of GS-6/7. These services will provide the served population information about VA Medical services, Benefits and Readjustment Counseling services at Vet Centers. The contract employee(s) will provide appropriate referrals and follow-up with the Vet Center on all referrals. These services will be provided by engaging in a variety of community outreach activities that will promote knowledge and use of Vet Center services and other VA services. These activities will include (but not be limited to) such items as public speaking, professional presentations, information booths, liaison with community and government agencies, interviews with the media and personal telephonic contact with the target population. Scope of Work: The contractor will provide one or more peer and culturally competent employees dedicated full time to carrying out the duties of this SOW in conjunction with the host Vet Center designated for this project. The contractor will carry out the required transition services in close coordination with the host Vet Center outreach and counseling services. The contractor will be responsible for all costs associated with this contract to include office, office supplies, local transportation needs and distance travel, employee(s) salary and benefits, communications i.e. phone and computer access as needed. Performance period: The period of performance is base year for 12 months from date of award with two one year options. Type of Contract: The contract will be a firm-fixed price agreement. Estimated costs are $100,000.00 per year for each contract awarded under this pilot program. Responsibilities of the Contractor Task 1: The contractor employee(s) dedicated by contract will be a peer (OEF/OIF veteran(s)) who is /are culturally competent. They will provide to the targeted veterans information on Vet Center and VA Services and benefits, outreach and preventive educational services to combat veterans and family members in the community/area designated by the contract. The contract employee(s) will assure that the outreach and preventive educational services provided are closely coordinated with the services and activities of the host Vet Center. Deliverable: The contractor will provide the resume(s) of the employee(s) that will provide the services to ensure that they are peers and culturally competent. The employee(s) dedicated by contract will meet with the Team Leader and COTR (may be the same person) of the host Vet Center on a mutually agreed on schedule, but at least monthly, to report the number of Veterans provided with outreach services and the total number of family members contacted. Veteran outreach contacts will be documented by the last four of the Veteran's social security number. During these meetings they will coordinate their outreach activities and services with the host Vet Center to ensure coordination of activities. Task 2: The contractor shall provide a detailed strategic outreach plan identifying areas of Veteran population and local community events where Veterans and family members would likely attend. The plan should also include information about the geographical spread and demographic composition of the local Veteran population. This plan is to be submitted once at the beginning of the contract and intended to function as a strategic plan of action for the remainder of the year. The plan can be adjusted with the approval of the contractor and the COTR with concurrence from the Contracting Officer. Deliverable: A detailed outreach plan and briefing shall be provided to the COTR within 15 days after award of contract. Task 3: The contractor employee(s) shall make referrals to the Vet center for follow-up readjustment counseling services as indicated during an outreach event. The contractor employee(s) will also make referrals to VA medical services, VA benefits and/or community agencies for needed and eligible services. Deliverable: The contractor employee(s) shall report to the Team Leader COTR any Veteran and or family referrals made within 24 hours of making the referral. The contractor employee(s) will also provide the Team leader COTR with any relevant contact information regarding the veteran or family member(S), where the Veteran and/or family member(s) were referred and the information that lead to the referral. This is particularly required for Veteran referrals deemed at risk of crisis. Task 4: The contractor or contractor employee(s) will provide the Team Leader COTR with monthly aggregate activity reports. Deliverable: The monthly report will include the total number of outreach events with a brief description of each, the number of Veteran outreach contacts, the number of family members contacted and the number of Veterans referred. The number of veterans referred will contain the name, last four of the Veterans social security number, where referred and if any follow-up was completed. The contractor will also after the first report provide an aggregate report in addition to the monthly report cumulative to date for the current year. Responsibilities of RCS (Team Leader/COTR) The RCS Team Leader/COTR will be responsible for the coordination with the contractor employee(s) in the provision of outreach services to the targeted veteran population. The Team Leader/COTR will meet with the contractor employee(s) monthly, will receive any referrals made and provide Vet Center follow-up as requested. The Team Leader/COTR will provide appropriate follow-up services in a timely manner for any referrals of veterans deemed to be in crisis. The Team Leader/COTR will review and accept or reject the written monthly reports from the contractor and the contractor's representative. The Team Leader/COTR will meet with the contractor and or contractor employee(s) to receive and review the detailed outreach plan and briefing within 15 days of award of the contract or other mutually agreed upon date. Contractor Experience Requirements: The incumbent(s) assigned under this contract should be well versed in military service and procedures for working with the military to include Reserve and National Guard personnel. Incumbent(s) should also have knowledge of community outreach and referral procedures. The contractor providing services under this contract should be OEF/OIF veteran(s) and native members from the population/area to be served under this contract. Confidentiality and Nondisclosure: As a VA business associate, the contractor shall be bound by all the same federal privacy laws as the VA. For the purpose of implementing this contract, the contractor shall maintain no records containing Veterans' personally identifying information.
