Description

The Department of Veterans Affairs Medical Center, Hampton, Virginia (VAMC Hampton) has a need for a contractor(s) to provide Healthcare for Homeless Veterans Temporary Rehabilitative Residential Housing Services for whom such care is specifically authorized by the Department of Veterans Affairs Medical Center (VAMC) in accordance with the Performance Work Statement in the solicitation package. This includes the facility, staffing, equipment supplies and services. The Contractor shall provide housing to include, but not limited to beds, restrooms, bath, kitchen facilities, laundry facilities and related furnishings. Contractor shall also provide food daily for three balanced meals to include a daily snack as well as personal hygiene items, first aid supplies and storage as needed, transportation to and from the VAMC and other appointments as required. Other services required by the Contractor includes but are not limited to: wound care, ADL assistance, daily plan of care, structured group activities to include physical activities, monitoring of medications, supportive social services and basic case management services and assistance in learning and development of responsible living patterns. All Contractor staff members are required to be trained and competent in administering breathalyzer tests. It is understood that the type of beneficiaries to be cared for under this contract will normally require care and services over and above the level of room and board. The number of beds required is four. Government anticipates award of a commercial item contract to the responsible offeror whose offer provides the best value to the Government. This will be an Indefinite Delivery, Indefinite Quantity (IDIQ) type contract and this acquisition is unrestricted. The size standard for North American Industry Classification System (NAICS) code is 624221. The size standard is $7.0 Million. The procurement will be conducted utilizing the commercial item procedures of FAR Part 12 in conjunction with FAR Part 15. Solicitation packages will be available on or about August 5, 2011. Proposals will be due into this office August 19, 2011. No hard copy solicitations will be provided. Solicitation documents may be obtained electronically at www.fedbizopps.gov.
