- STATEMENT OF WORK SUPPLY AND DELIVERY OF DRY ICE (CARDICE) TO THE DEPARTMENT OF VETERANS AFFAIRS, VETERANS HEALTH CARE SYSTEMS OF THE OZARKS (VHSO), MEDICAL CENTER, 1100 N. COLLEGE AVENUE, FAYETTEVILLE, ARKANSAS FOR A ONE (1) YEAR PERIOD, WITH OPTIONS TO EXTEND FOR UP TO AN ADDITIONAL FOUR (4) YEARS. Background/ Scope The Department of Veterans Affairs, Veterans Health Care System of the Ozarks (VHSO), VA Medical Center, Fayetteville, Arkansas requires Dry Ice (Cardice) for the preservation of frozen specimens sent for processing to other Veterans Affairs Medical Centers (VAMC) facilities. The Dry Ice shall be delivered weekly to the Fayetteville Medical Center Laboratory in 250 pound shipments. Delivery must be on Monday (Tuesday, if Monday is a holiday) before 12 Noon. The Contractor shall maintain an insulated Dry Ice Storage Box in the Fayetteville Medical Center Laboratory at NO additional charge for storage of the Dry Ice. The Proposed Unit Price shall be an all inclusive price that includes HazMat and Energy charges.
