- Sources Sought: This is a sources sought announcement only. This is not a solicitation announcement. The purpose of this announcement is to gain knowledge of current market capabilities in relation to NAICS Code 622211, Office of Physicians, Size Standard $9.0 Million. The Network Contracting Activity at Bath (VISN 2) has a requirement for a contractor to provide outpatient, off-site colonoscopy services for veterans. Contractor must be able to provide an all inclusive proposal including professional services, equipment, facility, nursing and technical personnel, drugs, supplies and pathology. Contractor ™s facility must be located within the Bath catchment area (approximately 90-mile radius). Responses to this sources sought must include the following information. 1. General information of your company name, address, contact person name, phone number, fax number, email address(s) and DUNS number and company website if available. 2. Type of business such as 8(a), small business, Hub Zone small business, Small Disadvantaged Business, Service Disabled Veteran Owned small business or Veteran owned small business. 3. Company experience with providing outpatient colonoscopy services. Responses to this market Survey should be in writing and shall be faxed or emailed to the attention of Jeanette Crooks, Contract Specialist at fax number 585-393-7883 or e-mail Jeanette.Crooks@va.gov. No telephone responses will be accepted. Responses must be received no later than 3:00 pm August 4, 2011. No solicitation document is available at this time; this notice is to acquire information only. Vendors interested in providing an offer will have to respond to a separate solicitation announcement that shall be published separately.
