Description
THIS IS A SMALL BUSINESS SOURCES SOUGHT NOTICE REPRESENTING A MARKET SURVEY AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS The Department of Veterans Affairs is conducting a sources sought to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are Service-Disabled Veteran-Owned, Veteran-Owned, Small Business, HUB Zone 8(a), Women-Owned, or small disadvantaged business concerns; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice. Small businesses concerns shall be capable of provide the necessary equipment and personnel to furnish service in the volume required for all the items under this contract. Contractor shall meet all requirements of Federal, State or City codes regarding operations of this type of service. The NAICS code is 623110 (Size Standard: $13.5 million) DESCRIPTION OF THE REQUIREMENT: 1. Title of Project: Feeding of no more than but could be less than 20 Veterans from the Ralph H. Johnson Medical Center, Charleston, SC. 2. Scope of Work: The contractor shall provide all labor, supplies, food and snacks no more than but could be less than 20 Veterans from the Community Living Center at the Ralph H. Johnson Veterans Medical Center. 3. Background: "Contractor shall be certified by South Carolina Department of Health and Environmental Control (SC DHEC) and met Joint Commission on Accreditation of Healthcare Organizations (JCAHO) requirements. "Contractor shall have technical training and documented records of working experience in food preparation, sanitation inspections and its components within healthcare, educational and commercial facilities for a minimum of one (1) continuous year. "Contractor Food Service staff shall have Serve Save food safety training certifications or on the job class room type training. With training certifications submitted to VA Contracting Officer. "Contractor shall be equipped with all necessary equipments and personnel to perform the contracted work safely, effectively and timely. "Contractor has provide copies of any patient satisfaction surveys done in regards to patients food service to the Contracting Officer for Nutrition & Food Service, along with action plan if necessary. "Contractor has send two copies of completed written report to the VA Contracting Officer within seven (7) calendar days of SC DHEC or JCAHO inspection, with corrective action plan if there is a deficiency reported. "Contractor will report any deficiency that is deemed critical or catastrophic immediately to the Contracting Officer and Contracting Officer for Nutrition & Food Service immediately for action. "Contractor shall have ready access o the latest versions of the following references: oSouth Carolina DHEC oJCAHO Manual oFood and Drug Administration manual oSC Approved Diet Manual 4. Performance Period: This contract is for the period of October 1, 2011 - September 30, 2012 with an extension clause for up to one (1) year. 5. Type of Contract: This is a firm-fixed-price contract. oMeals will be served at approximately 7:15-7:45 AM Breakfast, 11:15 - 11:45 AM Lunch and 5:15-5:45 PM Dinner.* (* must be 14 hours between dinner & breakfast) oMeals may be requested for alternate meal times under special circumstances, i.e.; Veteran not feeling well, Veteran off premises for appointments and family gatherings. oSnacks shall be available on the unit 24 hours/day 7 days/week and replenished daily to a par level to assure Veterans have access to "foods/snacks/drinks whenever they want/whatever they want" - per VISN Director. oCold bagged breakfast will be supplied to Veterans who must travel to RHJ Medical Center for dialysis in a timely manner. (Apple juice, cornflakes or rice krispies, milk, spoon, sugar or sugar sub, biscuit or muffin, coffee, cream, bottle of water) oVeterans' birthdays will be acknowledged with a birthday cupcake or a special meal for Veteran and their family member(s) - must be ordered three days in advance. (menu inserted here) oVeterans' food preferences will be honored or selective menus completed to assure Veterans are receiving foods they prefer to consume. (insert sample menus here) oMeals must follow the VA guidelines. oAbbot Lab Tube feeding products will be provided. (Ensure, Ensure Plus, Jevity, Nepro, Glucerna, ) POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large) and DUNS Number. 2) Provide a Statement of Capability that demonstrates the offeror's past performance in providing this type of service. Please include the following: (a) staff expertise including their availability, experience, and formal and other training; (b) current in-house capability and capacity to perform the work; (c) prior completed projects of similar nature; (d) corporate experience and management capability; and (e) examples of prior completed Government contracts, references, and other related information. 3) Submit Capability Statements by August 3, 2011 to: Department of Veterans Affairs, VISN 7 Network Contracting Activity ATTN: Juan A. Jusino and Amy Davis, Augusta, GA 30904-6285. Responses should be received no later than 2:00 PM EST. Submissions must be made via email to: juan.jusino@va.gov, Amy.Davis5@va.gov or FAX: (478) 274-5771 NO CAPABILITY STATEMENTS MAY BE SUBMITTED AFTER AUGUST 3, 2011. AT THIS TIME NO SOLICITATION EXISTS. (DO NOT REQUEST A COPY OF THE SOLICITATION). A FUTURE NOTICE WILL BE POSTED WHEN THE SOLICITATION WILL BE RELEASED. 4) Disclaimer and Important Notes: This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published in Federal Business Opportunities. However, responses to this notice will not be considered adequate responses to a solicitation.
