- Statement of Work Attachment 2: Wage Determination (2005-2131 Rev. 12 dated 13 Jun 2011) Attachment 1: Pricing Schedule This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. The award resulting from this combined synopsis and solicitation will be a Firm Fixed Price purchase order for the items listed. The solicitation number for this procurement is FA4830-11-Q-0061 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-53, Defense Acquisition Circular (DAC) Correction 91-13 and Air Force Acquisition Circular (AFAC) 2011-0714. This acquisition is intended to be sole-source; however, businesses that are identifed as Small Business, HUBZone, 8(a), and Service Disabled Veteran Owned Small Business for NAICS code 541511 are being sought as sources. The North American Industry Classification System (NAICS): 541511. Quotations must be submitted on the attached Pricing Schedule for the following CLINS. CLIN 0001: Siemens, Part # MOS24716, Essential On - Site Maintenance 24x7 Legacy Systems, Comscendo Plus Lics - OS247 (168 EA) CLIN 0002: Siemens, MOS24735, Essential Maintenance 24x7, Miscellaneous Applications, Products, Products, and Fees, HiPath Trading - OS247 (3 EA) CLIN 0003: Siemens, PHOS, Essential On-Site Maintenance, OSV, Phone Replacement On-Site (168 EA) CLIN 0004: Siemens, SIFA Radio Interface (3 EA) CLIN 0005: Siemens, PTT Handsets/headsets (6 EA) CLIN 0006: Siemens, PTT Closure Box (3 EA) CLIN 0007: Siemens, PEC400SC - HP Trading Server (1 EA) CLIN 0008: Siemens, 1707FP 17" LCD for HP Server (1 EA) CLIN 0009: Siemens, RAD OP-108 (T-1 to Fiber Modems) (2 EA) CLIN 0010: Siemens, 23-ch DAKS Server, Rel. 6.1 (1 EA) CLIN 0011: Siemens, 16-ch NICE Logger Silver Plan M-F 8-5 (1 EA) CLIN 0012: Siemens, APC RT3000 w/ 3 batteries (1 EA) EVALUATION/AWARD: IAW FAR 52.212-2, Evaluation-Commercial Items, the Government will award a purchase order to the quoter whose price is judged to represent the best value to the government. Evaluation criteria are as follows Lowest Price Technically Acceptable. Technically acceptability will be based on the contractor meeting qualifications and performance specifications in accordance with the Siemens Maintenance Statement of Work as attached. The quote which conforms with the government's specifications and is most lowest-priced will be deemed the "lowest price technically acceptable" quote. Quoters MUST BE REGISTERED ON CENTRAL CONTRACTOR REGISTRATION (CCR). Information concerning CCR requirements may be viewed via the internet at http://www.ccr.gov or by calling the CCR Registration Centers at 1-888-227-2423. Place of Performance: Moody AFB, GA Period of Performance: Base and four option years Provision at FAR 52.212-1, Instruction to Offerors Commercial Items applies to this acquisition. Request for Quotation (RFQ) should be submitted and shall contain the following information: RFQ NUMBER; TIME SPECIFIED FOR RECEIPT OF QUOTES; NAME, ADDRESS; TELEPHONE NUMBER OF QUOTER; TERMS OF THE EXPRESSED WARRANTY; ANY DISCOUNT TERMS AND ACKNOWLEDGEMENT OF ALL SOLICITATION AMENDMENTS (if applicable). Signed quotes must indicate quantity, unit price and total amount for each item. Quoters shall include a statement specifying the extent of agreement with all terms, conditions and provisions included in the solicitation. Quotes shall also contain all other documentation specified herein. PERIOD OF ACCEPTANCE OF QUOTERS: The quoter agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation. LATE QUOTES: Request for Quotations or modification of quotes received at the address specified for the receipt of quotes after the exact time specified for receipt of quotes WILL NOT be considered. All quoters must be registered on Central Contractor Registration (CCR) and online representation and certification application (ORCA) must be activated. ORCA registration for on line representation and certification requirements may be viewed via the internet at www.bpn.gov.
