Acquisition Management Division, Logistics Department, NNMC Laboratory Department, NNMC STATEMENT OF WORK FOR INTERIM COURIER REQUIREMENT FOR LABORATORY SERVICES Request two month interim contract for laboratory courier services in lieu of the main contract N0016811RC05467 currently in progress at FISC Philadelphia. The main contract is on pace for award on 15 September 2011 for implementation on 29 September 2011. The Laboratory's mission requires these services to begin no later than 01 August 2011 in order to maintain regulatory and accreditation requirements. Required performance characteristics of this courier contract are as follows: 1. Stat pick-up and delivery may include both one-way and round-trip services. The northern boundary of the range for stat pick-up and delivery is expected to be but not limited to the American Red Cross, 4700 Mount Hope Drive, Baltimore, MD 21215. The Laboratory reserves the right to re-prioritize stat pick-up and delivery services in the event an additional need arises while the courier is enroute to a pick-up. Should a second courier not be available the Laboratory must be able to give the courier instructions as to what pick-up is of the highest priority. This occurrence is expected to be minimal in the lifecycle of the contract. The Courier will routinely transport: Laboratory samples to include but not limited to: Blood and bodily fluids in varying amounts Tissue cassettes submerged in 10% Formalin Stained Tissue Glass Slides Cytology Specimens in methanol and Cytology Slides Culture Swabs Bacterial and Viral Cultures Blood products for transfusion (estimated volumes of sample transport for hazardous materials are as follows: approximate maximum of 100-150 surgical samples in 30 ml of 10% buffered formalin transferred from East Campus to North Campus, up to 300 samples for the testing of HPV in methanol based solution transferred between campuses, and the transport of up to 300 tissue cassettes in up to 1 liter per 100 cassettes of 10% buffered formalin.) Additional Items routinely transported are to include but not limited to: Administrative Paperwork Reagents and laboratory supplies Vendor requirements : •- Certifications and or any other requirements which the vendor will need to access the applicable government installations. •- Vendor must be able to transport and handle small amounts of HAZMAT, primarily alcohols, neutral buffered formalin and category A and B infectious substances. •- Must be knowledgeable of all State Laws and Regulations in the Baltimore-Washington Metro Area required for the transportation of HAZMAT. •- Provide personnel who are legally allowed to work in the US. •- Personnel must have a legal Driver's license from State of Origin. •- Vehicle must be fully Insured and in compliance with all laws of Maryland, Virginia, and the District of Columbia. •- Vendor is required to comply with all applicable base parking and zoning requirement which may require transportation of items some distance from vehicle to building. •- Vendor is required to supply dry ice for frozen specimens daily with no additional cost to the government. •- Each set of samples must remain at the intended temperature range it was pack at throughout the transportation ie, frozen or dry ice, refrigerated or wet ice/cold pack, and room temperature. •- The courier will contact the destination facility if traffic or other concerns cause a delay of more than 20 minutes. •- The courier service must provide 24 hour, 7 days per week telephone support for STAT run dispatch. • - The courier will be on site for stat pick up requests within 60 minutes. The Government will provide: •- Sample transportation carriers •- Hand truck •- Point of Contact for each facility to include Name, phone number, and e-mail. Point of contact is responsible for notifying the courier service of changes to the facility parking and traffic patterns with a minimum of 72 hours or the maximum notice given by facility/base security personnel. •1.
