- CLIN DETAIL (i) This is a combined synopsis/solicitation prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and written solicitation will not be isssued. Anticipated award date is 15 July 2011. (ii) This solicitation/synopsis reference number N68470-11-T-0228 is issued as a Request for Quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-52 dated 01 May 2011. (iv) NAICS Code for this requirement is 339113. (v) The U.S. Naval Hospital, Okinawa, Japan intends to negotiate a sole source procurement under he authority iof FAR 13.106-1(b) with Depuy Orthopaedics for DVR Anatomic implants. All responsible sources may submit a quotation which shall be considered. See attachment for the Schedule of Supplies. (vi) Contractor must be registered in Central Contractor Registration, FAR 52.204-7 [Note: Lack of registration in the Central Contractor Registration will make a Contractor ineligible for award] Registration is available via the Internet at www.ccr.gov. Delivery FOB Destination to U.S. Naval Hospital (USNH)- Okinawa, Japan BLDG 6000 Camp Lester Chatan Cho Kuwae, Okinawa, Japan 904-0103 by 12 Aug 2011. Origin And/or Destination Evaluation; 52.232-33 Payment by Electronic Funds Transfer - Central Contractor Registration; DFARS 252.204-7004 Required Central Contractor Registration; DFARS 252.232-7003, Electronic Submission of Payment Requests; 252.243-7001 Pricing Of Contract Modification; 252.212-7001 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items; DFARS 252.204-7000 Disclosure Of Information.
