- REQUEST FOR INFORMATION/SOURCES SOUGHT NOTICE Please note this is not a request for quotes or proposals; the Government is seeking information for market research purposes only. Your responses to the information requested will assist the Government in determining the appropriate acquisition method. A solicitation is currently not available; however, the Government may issue a solicitation in the near future. The Philadelphia VA Medical Center (PVAMC), located at 3900 Woodland Avenue, Philadelphia, PA 19104, is seeking potential sources to provide a (Demonstration Unit) OrthoScan-HD Mini C-Arm System under the established NAICS Code 334517 "Irradiation Apparatus" to include the following: 1. Orthoscan-HD Mini C-Arm System (Demonstration Model only) 2.Digital Printer 3.DICOM for PACS Server 4.Digital Video Recorder 5.Wireless Multi-Function foot switch 6.12 month Warranty-All parts and labor 7.One Day on-site in-service training 8.Shipping, Handling, Installation 9.Delivery 30 days after date of the awarded order If your company is interested and properly qualified, please provide the following information: Name and address of company; DUNS number; CAGE code; GSA contract number (if applicable); capability statement; business size; socio-economic category of your firm (including Service-Disabled Veteran-Owned Small Business, Veteran-Owned Small Business, Woman Owned Small Business, Small Business, Large Business, Other than Small Business); and any other pertinent information.
