- The VA Salt Lake City Health Care System has a requirement for Sleep Study Testing of Beneficiaries in the Salt Lake City, Utah area. This Pre-solicitation notice replaces VA-259-11-RP-0014. The Contractor shall perform Nocturnal Polysomnographic (NPSG) testing at the Contractor's facility. VA beneficiaries shall be referred from the Salt Lake VA Medical Center, Salt Lake City, Utah. Approximately 600 tests per year will be required. Contractor shall provide a diagnosis and report of the study at its completion. The proposed term of this contract shall be for a base year with provisions for four-one year option periods. The North American Industry Classification System number for this procurement is 621498. The Small Business Size for this procurement is $10m. Solicitation packages will be available electronically on or about July 15, 2011. Receipt of offers will be on or about August 16, 2011. All prospective offerors desiring to submit proposals in response to this or any Federal Agency RFQ or RFP must obtain a Dun and Bradstreet Number at www.dnb.com. There is no cost to obtain this number. Offerors must subsequently register with the Central Contractor Registration (CCR) at www.ccr.gov. This acquisition is 100% set aside for Small Business.
