Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This announcement constitutes the only solicitation; proposals/quotes are being requested and a written solicitation will not be issued. This is not a total small business set-aside. The associated NAICS code is 541690 and the small business size standard is $7Million. The small Business Competitiveness Demonstration Program is not applicable. This solicitation is issued as a Request for Quote (RFQ). The Collateral Duty Safety Training Course will be held at the Installation Safety Office, Building 3687, Redstone Arsenal, AL. The Statement of Work for this requirement is as follows: Statement of Work For Collateral Duty Safety Training 1.0 SCOPE. The contractor shall furnish all labor, materials, equipment, transportation, tools, supervision, and any other items necessary to provide a collateral duty training program that shall be used to train individuals on how to recognize workplace hazards. 2.0 APPLICABLE DOCUMENTS. The Army Safety Program 3.0 REQUIREMENTS. 3.1 The contractor shall present a three-day course on the requirements, duties and responsibilities of becoming a Collateral Duty Safety Officer (CDSO) IAW AR 385-10. The contractor must provide the training prior to October 2011. 3.1.1 The contractor shall train students in the use of the Composite Risk Management process. 3.1.2 The contractor shall train students to complete a risk assessment worksheet. 3.1.3 The contractor shall train students to complete an Army accident report. 3.1.4 The contractor shall train students to be able to conduct a safety inspection and complete a Hazard Log. 3.1.5 The contractor shall train students to be able to assist leaders and supervisors with Occupational Safety and Health Act (OSHA) compliance. 3.2 This curriculum shall comprise of the course overview, Army Safety Program, Accident Causation, Duties and Responsibilities, Composite Risk Management, Introduction to OSHA Responsibilities, Safety and Health Programs, Egress, Accident Investigation, Reporting and Recordkeeping, Walking/Working Surfaces, Hazard Communication, Material Handling and Storage, Ergonomics, Personal Protective Equipment (PPE), Hearing Conservation, Machine Guarding, Lockout/Tagout, Electrical Safety, Confined Space and how to conduct a worksite survey. 3.3 Redstone Arsenal will not be responsible for any expenses involved in presenting any vendors products. 3.4 Training location: Installation Safety Office, Building 3687. 3.5 Period of Performance: The contractor shall accomplish this work between the hours of 0730 and 1630 hours. 4.0 Contractor Manpower Reporting. The Office of the Assistant Secretary of the Army (Manpower & Reserve Affairs) operates and maintains a secure Army data collection site where the Contractor shall report ALL Contractor manpower (including subcontractor manpower) required for performance of this contract. The Contractor is required to completely fill in all the information in the format using the following web address: https://cmra.army.mil. The required information includes: 1. Contracting Office, Contracting Officer, Contracting Officer's Representative; 2. Contract number, including task and delivery order number; 3. Beginning and ending dates covered by reporting period; 4. Contractor name, address, phone number, e-mail address, identity of Contractor employee entering data; 5. Estimated direct labor hours (including sub-contractor); 6. Estimated direct labor dollars paid this reporting period (including sub-contractors); 7. Total payments (including sub-contractors); 8. Predominant Federal Service Code (FSC) reflecting services provided by contractor (and separate predominant FSC for each sub-contractor if different); 9. Organizational title associated with the Unit Identification Code (UIC) for the Army Requiring Activity (the Army Requiring Activity is responsible for providing the Contractor with its UIC for the purposes of reporting this information). 10. Locations where Contractor and subcontractors perform the work (specified by zip code in the United States and nearest city, country, when in an overseas location, using standardized nomenclature provided on website). 11. Presence of deployment or contingency contract language. 12. Number of Contractor and subcontractor employees deployed in theater this reporting period (by country). As part of its submission, the Contractor will also provide the estimated total cost (if any) incurred to comply with this reporting requirement. The following provisions apply to this acquisition: 52.212-1, Instructions to Offerors -- Commercial 52.212-4, Contract Terms and Conditions -- Commercial Items 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, The offeror must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. 252.232-7003 Electronic Submission of Payment Requests and Receiving Reports. 252.212-7001 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items.
