- RFP VA-528-11-RP-0222 is being issued to assist with the provision of residential placement and supportive services through the Health Care for Homeless Veterans (HCHV) program. The VA Health care Network Upstate New York (VISN 2) has been tasked with finding a Provider/Offeror in the following areas in New York State: Albany, Bath, Buffalo, Canandaigua and Syracuse, with experience and proven record of successfully working with residents recovering from co-morbid conditions such as serious mental illnesses, substance abuse, disorders, and other serious medical conditions. The primary goal is to develop alternative housing for the Veterans and to prepare them for independent living. It is the ultimate goal that these Veterans will eventually find their way to housing located within the community. The NAICs code for this is 624229, Other community housing services (size standard of $7.0 million). Registration (CCR) at http://www.ccr.gov is required in order to be eligible for award of Government contracts.
