Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. SPECIFICATIONS: The USPFO for Utah has a requirement to purchase the following services: Catered meals for 11-16 July, 18-23 July, 25-27 July 2011 1.Breakfast - Qty 75 each daily (grand total 1125). Consisting of all hot meals served at 7:00 am delivered to either: Camp Williams in Riverton Utah, Global One Training Facility in Fairfield, UT, or Swanson Tactical Training in North Ogden, UT. Locations will vary between those three locations. Specific location details will be included in contract award. 2.Lunch - Qty 75 each daily (grand total 1125). Boxed/sack lunches delivered WITH breakfast in coolers. Coolers will be returned at dinner service. 3.Dinner - Qty 75 each daily (grand total 1125). Consisting of all hot meals served at 6:00 pm delivered to either: Camp Williams in Riverton Utah, Global One Training Facility in Fairfield, UT, or Swanson Tactical Training in North Ogden, UT. Locations will vary between those three locations. Specific location details will be included in contract award. REQUIREMENTS: 1.Quotes will only be accepted from local suppliers. 2.Proposed menus MUST be submitted with quote. 3.Menus MUST meet a combined daily caloric intake of 3500-4000 per person. 4.Beverages provided must be bottled water, electrolyte drinks, milk, or juice. No sodas permitted. 5.Quotes must meet the following criteria: a.Unit costs per breakfast, lunch, dinner, and delivery fee. b.Include quotation number, offerors name and address, point of contact, phone number, e-mail address, CCR number, and TIN on cover sheet submitted with bid sheet. Failure to provide this information with your quote may cause your offer to be considered non-responsive. QUOTE DUE DATE: NLT 10:00 am MST 24 June 2011. Quotes may be e-mailed to jennifer.shaw5@us.army.mil or mailed to USPFO for Utah, Attention Jennifer Shaw, 12953 South Minuteman Drive, Draper, UT 84020-2000. Facsimiles will NOT be accepted. All quotes must arrive at the place and by the time specified. ADDITIONAL INFORMATION: 1.Quote will be awarded based on lowest price technically acceptable. 2.Catered meals Army and Utah National Guard Regulations: AR 30-22, 3-41a. Catered meals A catered meal is defined as "a meal prepared in a commercial establishment for consumption there or delivered to a specific location for consumption or, prepared at the consumption site using caterer's portable equipment and subsistence supplies." There is not authority for a caterer to use Government furnished equipment to prepare subsistence. UTNG Pam 30-22, 2-1d. Catered meals: Catered meals are defined as a meal that is prepared off-site and delivered to a feeding site for consumption. Caterers are not authorized to use GFE or facilities to prepare food that is to be consumed by diners; however caterers may use hot and cold food serving equipment to hold and serve food. NOTE: You must receive "approval" from the Approving Authority. In accordance with the above references, caterers are not authorized to use any GFE or facilities to prepare meals for profit. This includes all UTNG Readiness Centers, Facilities at Camp Williams or DPG. 3.Included in this document is the Quality Assurance Surveillance Plan (QASP) For Catered Meals. The purpose of the QASP is to identify the methods and procedures the government will use to evaluate contractor actions and quality assurance while performing the requirements as stated in the contract. QASP's will aid in determining contractor's past performance and may impact future opportunities. APPLICABLE PROVISIONS AND CLAUSES: The following clauses and provisions are incorporated by reference. Thank you, "//SIGNED//" Jennifer Shaw Utah Army National Guard Contract Specialist Quality Assurance Surveillance Plan (QASP) For Catered Meals 1.0INTRODUCTION 1.1.1.Purpose. The role of the government in quality assurance is to ensure contract standards are achieved. The purpose of the QASP is to identify the methods and procedures the government will use to evaluate contractor actions while performing the requirements as stated in the Contract and/or Statement of Work (SOW). 1.1.2.The QASP provides a systematic method to evaluate the services the contractor is required to furnish. The QASP is based on the premise the government desires to maintain a quality standard for catered meals. 1.1.3.Personnel surveying the contract terms and SOW requirements will review the QASP throughout the life of the contract. 2.0.DESCRIPTION OF SERVICES 2.1.1.Scope of Work. The contractor shall provide all personnel, equipment, tools, materials, vehicles, supervision, and other items necessary to meet the service requirements as defined in the Contract and/or Statement of Work (SOW). 3.0.SURVEILLANCE 3.1.1 Surveillance Method. This QASP will evaluate the pre-defined criteria as shown on the following page to ensure the contractor complied with the SOW requirements. The Government Point of Contact (POC) as stated within the contract will conduct the evaluation.
