Description
Statement of Work for Asphalt Repairs Background The Medical Center Parking Lots K, D and E have areas where the asphalt has become damaged. Damage has been caused by: excavation for underground utilities; settling of the sub base material; some areas have been previously repaired; some damage is from weather. This project has been separated into Tasks for bidding purposes. Appendix A shows the VAMC site. TASK 1: Task 1 in Parking Lot E consists of approximately 220 sq. ft. located in approximately 6 locations requiring approximately 8 cubic yards, of damaged asphalt to be replaced.. These areas will need to be saw cut and excavated down to the sub base material removing all the damaged asphalt and then replaced. It is believed the asphalt in this area is approximately 4 inches thick. Appendix B shows the location of Task 1 on the VAMC site. TASK 2: Task 2 in Parking Lot D consists of approximately 280 sq. ft. of damaged areas approximately 9 locations containing requiring approximately 11 cubic yards of damaged asphalt to be replaced. These areas have asphalt damaged and missing, exposing the sub base material. These areas will need to be saw cut and excavated down to the sub base material removing all the damaged asphalt, and then replaced. It is believed that the asphalt in this area is between 4 and 6 inches thick. Appendix C shows the location of Task 2 on the VAMC site. TASK 3: Task 3 consists of 2 locations requiring work. The first work site is located at the north east corner of Building # 7. This location has an electrical hand hold box located just off the pavement. Currently, the box is slightly above the grade of the area and needs to be at the same level or slightly lower than the adjoining area to facilitate snow removal and grounds work in the area without damaging the Electrical hand hold box. This area is approximately 10 sq. ft. in size. The second work site in this area is in parking Lot K, a Handicap parking lot located on the west end of the VAMC site. The damaged area requiring repair is at the west end of parking lot B. This area consists of a depression in the pavement approximately 6 ft. wide by 40 ft. long totaling approximately 240 sq. ft., and approximately 12 cu. yds. This area will need to be saw cut and excavated down to the sub base material removing all the damaged asphalt and then replaced. It is believed that the asphalt in this area is approximately 4 inches thick. Appendices D and E show the locations of task 3 on the VAMC site. GENERAL GUIDANCE The sizes of all the identified areas to be repaired are approximated and may vary. The repairs to all the areas may include, but not be limited to; asphalt saw cutting, jack hammering, chiseling, grading, compacting, rolling, back filling, and laying asphalt with hand tools and motorizes equipment. All the areas to be repaired will require complete removal of all loose and damaged material. The areas will need to be properly prepared with back filling, compacting, rolling etc. before application of asphalt base coat and finish coat. OTHER CONSIDERATIONS FOR PROPOSAL 1.The sizes of all the identified areas to be repaired are approximated and may vary. The repairs to all the areas may include, but not be limited to; asphalt saw cutting, jack hammering, chiseling, grading, compacting, rolling, back filling, and laying asphalt with hand tools and motorizes equipment. All the areas to be repaired will require complete removal of all loose and damaged material. The areas will need to be properly prepared with back filling, compacting, rolling etc. before application of asphalt base coat and finish coat. 2.There will be one opportunity to visit the site and review the existing conditions. 3.The contractor will provide all the labor, material and supplies to complete this project. The contractor is responsible for all fees and permits necessary for the work of this project. 4.Perform work in compliance with the tasks above, the Contractor requirements (attached) as well as Specifications: 01 00 00 - General Requirement 02 41 00 - Demolition for Asphalt Repairs 32 12 16 - Asphalt Requirements 32 17 23 - Pavement Markings for Asphalt Repairs Section 413 - Hot-Poured Crack Sealant 5.The Contractor shall arrange for all waste to be recycled or disposed of in accordance with all Federal, State, or local laws and regulations. 6.Bidders must submit a copy of the following as part of their bid: "Safety Plan. "Lock Out / Tag Out Program. "Respiratory Protection Plan (if necessary). 7.Additionally, the following must be submitted and approved by the VA before the contractor begins work on site: "Activity Hazard Analysis (AHA). A blank AHA form is available at http://www.swf.usace.army.mil/pubdata/safety/PDF/blank_AHA_Form.pdf. "Schedule. "Work plan for executing the work of this contract to include road or parking closures. Two week minimum notice is required prior to any closure of parking areas. "Material Safety Data Sheets (MSDS) for any hazardous materials to be brought on site. NO chemicals may be brought on site until their MSDSs have been accepted. "30 hour OSHA Construction safety training for the site supervisor, and OSHA 10 hour training for all workers that area assigned to the project. 8.Allow 2 weeks for review of all submittals. 9.The provisions of all applicable NFPA, OSHA, EPA, and VA codes, Federal, State, and local regulations shall be followed throughout the course of this project. Applicable VA publications and design manuals are available to the successful bidder at www.va.gov/facmgt. 10.All utility shut downs shall be coordinated through the COTR, and written notice given at least two weeks in advance of the shut down. 11.The contractor will be required to provide competent traffic control persons at all times during any construction activity. This work must be completed with the least amount of disruption to the daily operation of the Medical Center and avoid any unnecessary inconvenience to patients, visitors and employees. 12.Normal working hours are Monday through Friday, 8 AM to 4:30 PM.
