- Self-Certification Form Terms & Conditions Identifies scope of work and dimensions of both basketball and soccer courts. Interested vendors must include update W-9 Form and Certificate of Insurance and if awarded project will need payment and performance bonds. Resurface and paint outdoor basketball and soccer courts. These courts are on concrete slabs and will need to be resurfaced painted and sealed with arylic paint. Contractor will include on soccer court - two (Noni Tip) portable soccer goals. Diagram will be available upon request. Attached construction bid package, scope of work has dimenisions of both courts.
